Yangetal.DiagnosticPathology2012,7:7 /content/7/1/7 CASE REPORT OpenAccess SecondarycutaneousEpstein-Barr virus-associateddiffuselargeB-celllymphoma inapatientwithangioimmunoblasticT-cell lymphoma:acasereportandreviewofliterature Qing-XuYang1,Xiao-JuanPei1,2,Xiao-YingTian3,YangLi2andZhiLi2* Abstract OnlyafewcasesofextranodalEpstein-Barrvirus(EBV)-associatedB-celllymphomasarisingfrompatientswith angioimmunoblasticT-celllymphoma(AITL)havebeendescribed.WereportacaseofAITLofwhichsecondary cutaneousEBV-associateddiffuselargeB-celllymphoma(DLBCL)developedaftertheinitialdiagnosisofAITL.A65- year-oldChinesemalepatientwasdiagnosedasAITLbasedontypicalhistologicalandimmunohistochemical characteristicsinbiopsyoftheenlargedrightinguinallymphnodes.Thepatientinitiallyreceived6cyclesof chemotherapywithCHOPregimen(cyclophosphamide,vincristine,adriamycin,prednisone),buthissymptomsdid notdisappear.NineteenmonthsafterinitialdiagnosisofAITL,thepatientwashospitalizedagainbecauseof multipleplaquesandnodulesontheskin.Theskinbiopsywasperformed,butthistimethetumorwascomposed oflarge,polymorphouspopulationoflymphocyteswithCD20andCD79apositiveonimmunohistochemical staining.ThetumorcellswerestrongpositiveforEBERbyinsituhybridization.Thefindingsofskinbiopsywere compatiblewithEBV-associatedDLBCL.CHOP-Rchemotherapy(cyclophosphamide,doxorubicin,vincristine, prednisoneandrituximab)wasthenadministered,resultinginpartialresponseofthediseasewithpancytopenia andsuppressionofcellularimmunity.Toourknowledge,thisisthefirstcaseofcutaneousEBV-associatedDLBCL originatedfromAITLinChinesepepole.WesuggestthepatientswithAITLshouldperformlymphnodeandskin biopsiesregularlyinthecourseofthediseasetodetecttheprogressionofsecondarylymphomas.
