The aim was to determine if SPARC, FOXP3, CD8 and CD45ROexpressionlevelsareassociatedwithcolorectalcancer(CRC)stage,diseaseoutcomeandlong-termcancer-specific survival(CSS)instageIIandIIICRC. MethodsandFindings:SPARCexpressionwasinitiallyassessedin120pairednormalandstageI-IVCRCs.Subsequently, approximately1000pairedpatientsamplesofstageIIorIIICRCsintissuemicroarrayswerestainedforSPARC,FOXP3,CD8 or CD45RO. Proportional hazards modelling assessed correlations between these markers and clinicopathological data, includingdiseaseoutcomeandcancerspecificsurvival(CSS).BothSPARCandFOXP3expressionweresignificantlygreater inCRCthannormalcolon(p,0.0001).HighSPARCexpressioncorrelatedwithgooddiseaseoutcome($60mthswithout diseaserecurrence,p=0.0039)andbetterlong-termCSSinstageIICRC(,0.0001).InstageIIICRC,highSPARCexpression correlated with better long-term CSS (p,0.0001) and less adjuvant chemotherapy use (p=0.01).
