肺癌晚期伴其他部位转移时的诊治体会 作者：罗南 李琼 赵夏 周克海 刘红兵 黄云 邹强 唐波 【关键词】 肺肿瘤 　　【摘要】 目的 探索对肺癌晚期伴有转移时的可行 治疗 方法 。方法 将2000年1月~2005年4月38例肺癌伴胸膜、胸壁、另叶肺、骨、肝、颅内转移的病人分成综合治疗、单纯手术、单纯化疗、单纯支持治疗四组进行治疗。结果 综合治疗组、单纯手术组、单纯化疗组、单纯支持治疗组中位生存期分别为15、10、11、6个月。1年生存率分别为7/9（77.8％）、4/8（50.0%）、5/10（50.0%）、0/11（0）。2年生存率分别为4/9（44.4%）、0/8（0）、0/10（0）、0/11（0）。结论 重视肺癌晚期患者自述固定疼痛部位的追踪检查，提高接诊时恰当推荐治疗方案的能力，把握对病人的依从性方面的处理技巧，改变原有的观念，对晚期肺癌PS 2级以上的病人在 经济 能支持、观念能接受时，采用多种治疗手段组合的综合治疗，对肺癌晚期伴有转移的病人延长生存期和提高生活质量是有益的。但何为最佳治疗组合顺序仍是一个值得探讨的 问题 。 　　【关键词】 肺肿瘤;肿瘤转移;诊断；治疗 　　Experience of the treatment to advanced lung cancer with metastasis in other position 　　【Abstract】 Objective To explore therapieutic approaches to advanced lung cancer with metastasis in other position.Methods From January 2000 to April 2005,38 cases of lung cancer with metastasis in pleura,thorax,other lobe of the lung,bone,liver and brain were allocated to 4 groups theated with 4 methods that integrated treatment,only operation,only chemotherapy and support treatment.Results The median survival time were 15,10,11,6 months and one-year survival rate was 7/9（77.8%）,4/8（50.0%）,5/10（50.0%）,0/11（0）;2year survival rate was 4/9（44.4%）,0/8（0）,0/10（0）,0/11（0） respectively.Conclusion It is benefit to prolong survival time and raise life quality if careful following-up is performed.It includes tracking fixed pain position of patients enhancing ability of persuasion and control patients that were treated,and selecting and using integrated treatment to patients with PS 2 over.But it is still worthy advanced lung cancer beyond to explore an optimum order in integrated treatment. 　　【Key words】 lung neoplasms;neoplasm metastasis;diagnose;treatment 　　2000年1月~2005年4月，我们对38例原发病灶在肺部，而后相继出现身体其他部位转移的肺癌病人进行了诊治，现将其体会简述如下。 　　1 临床资料 　　1.1 一般资料 38例病人，男26例，女12例，年龄43~72岁。根据病人所愿接受的治疗项目，分成综合治疗组、单纯手术组、单纯化疗组、单纯支持治疗组四种治疗类型。 　　1.2 各组治疗手段组合情况 （1）综合治疗组采用的治疗组合手段是：手术切除（根治和姑息方式），化疗（长春瑞滨或紫杉醇＋铂类，2~6个周期），放疗（体伽，头伽），中药（华蟾素，鸦胆子，回生口服液）治疗和放射性核素治疗。（2）单纯手术组采用的手段是根治和姑息切除方式，术中加用一次性铂类药局部化疗。（3）单纯化疗组的治疗采用的手段是第三代化疗药（长春瑞滨或紫衫
