· 804 · 中华实用诊断与治疗杂志 2008年第 22卷第 11期 论 著 危重婴儿先天性心脏病 围手术期应对策略* 林薇 ，谈凌凌，王哲 ，龚琪，徐振海 (摘要] 目的：探讨危重婴儿先天性心脏病 围手术期应对策略 。方法 ：138例危重先天性心脏病婴儿施行外科手 术 ，18例动脉导管未闭在非体外循环下行单纯结扎 ，12O例在体外循环下行心脏大血管畸形解剖矫治。结果：术后 呼吸 机 辅助 时间2～1l2h，平 均 (35．2±16．3)h，ICU 时间 2～7d，平均 (4．2±1．8)d，全 组住 院死 亡 8例 ，病 死率 5．8 (8／138)，其余术后 并发症 为低心输 出量综合征 12例 ，肺 不 张 5例 ，肺 部感 染 9例，均治愈 ，随访 1～24个 月，无晚期 死 亡 ，心功能 I～ Ⅱ级 。结论 ：危重婴儿先天性心脏病外科手术恰 当的围手术期综合处理是手术成功的保证 ，可挽救 多数 患儿生命 ，并取得 良好效果。 (关键词] 复杂先天性心脏病；婴儿；围手术期 中图分类号 ：R654．2 文献标识码 ：A 文章编号 ：1674—3474(2008)11-804—03 StrategyforcriticalandcomplexcongenitalheartdiseaseininfantsduringperioperationLIN W ei，TAN Lingling，W ANG Zhe，eta1．DepartmentofCardiothoracicSurgery，theFourthHospitalofHebei MedicalUniversity，Shijiazhuang050011，China [Abstract]0bjectiveTo studythefeasibility，indication，techniqueofoperation and the perioperationmanagementon criticaland complex congenitalheartdiseaseininfants．MethodsOne hundredandthirty—eightinfantswithcriticalandcomplexcongenitalheartdiseasereceivedoperation． inwhich 18receivedpatentductusarteriosuswithoutcardiopulmonarybypass，theother120cases wereoperatedwithcardiopulmonarybypass．ResultsThelastingtimeofintubationwasfrom 2to112 hours，averagely (35．2±16．3)hoursandICU stayingtimewasform 2to7days，averagely (4．2± 1．8)days．Theoperativemortalitywas5．8 (8／138)．Thepostoperativecomplicationsinother infantsincluded12casesoflow cardiacoutputsyndrome，5ofatelectasis，and9oflunginfection．All werefollowedupfrom 1to24months．Thecardiacfunctionwasfrom class工 to Ⅱ／Il1．The operativeresultwassatisfactory．Conclusion Theproperperioperationmanagementisimportantfor criticalandcomplexcongenitalheartdiseaseininfants，and can savemostoftheirlives．Thegood resultcanbeobtainedfrom surgicaltreatment． [Keywords3Complexcongenitalheartdisease；infant；perioperation
