胶体金免疫法粪隐血阴性结果研究处理[摘要] 目的：在临床检验工作中发现部分胶体金免疫法粪隐血阴性结果与临床诊断不符，其阴性结果有待于进一步分析处理。方法：将临床工作中胶体金免疫法粪隐血阴性标本分成外观正常组和异常组两组，用匹拉米同化学法进行复检，以发现免疫法中的假阴性结果。结果：胶体金免疫法粪隐血阴性颜色正常组与化学法匹拉米同复检结果一致，而胶体金免疫法粪隐血阴性颜色异常组与化学法存在较大差异。结论：对胶体金免疫法粪隐血阴性而颜色异常时需结合运用化学法进行复检，以避免假阴性结果。 [关键词] 粪隐血试验；胶体金免疫；假阴性；匹拉米同 [中图分类号] R446.13 [文献标识码] C[文章编号] 1674-4721（2011）03（b）-085-02 Analysis and processing of colloidal gold immunoassay fecal occult blood-negative results CHEN Tinghong Department of Medical Laboratory, Benniu Hospital of Wujin District, Jiangsu Province, Changzhou 213131, China [Abstract] Objective: In the clinical laboratory work, we had found that the results of some of colloidal gold immunoassay fecal occult blood-negative were different from those of the clinical diagnosis, and its negative results needed to be further analysed and processed. Methods: In clinical work, colloidal gold immunoassay fecal occult blood-negative samples were divided into normal and abnormal appearance of two groups, and Pyramidon chemical re-examination conducts a review to find false negative results of immunosorbent assay. Results: Colloidal gold immunoassay fecal occult blood-negative group with normal color was consistent with pyramidon results with chemical re-examination, but colloidal gold immunoassay fecal occult blood-negative group with the abnormal color was quite different from chemical method. Conclusion: Colloidal gold immunoassay fecal occult blood negative for the abnormal color needs to combine chemical methods to re-examine, It can conduct a review in order to avoid false negative results. [Key words] Fecal Occult blood test; Colloidal gold immunoassay; False-negative; Pyramidon 粪便隐血试验是诊断胃肠道出血性疾病的一个重要指标，当胃肠道少量出血时，粪便外观可无明显变化，肉眼不见血色。因少量红细胞被消化分解，以致显微镜下也无从发现，这种肉眼和显微镜均不能证实的出血称为隐血。隐血试验可作为消化道恶性肿瘤普查的一个筛选指标，其连续检测对早期发现结肠癌、胃癌等恶性肿瘤有重要的价值[1]，因此隐血试验成为人们不断深入研究的热点。 胶体金免疫层析法，将单克隆技术与胶体金技术结合，利用抗人血红蛋白或抗人红细胞基质的单克隆抗体与人血红蛋白或人红细胞有高度特异性的特点检测粪便隐血。由于不受饮食限制，灵敏度和特异性高，反应快速，血红蛋白为0.2 μg/ml时即可阳性[2]，在临床得到大力推广。但在实际应用
