重庆市儿童乙型肝炎病毒感染及治疗现状分析 谭顺，谭文婷，向小梅，刘明，毛青，邓国宏（400038重庆第三军医大学西南医院全军感染病研究所，感染病研究重庆市重点实验室） [摘要] 目的了解我院就诊儿童乙型肝炎病毒( epatitis B virus, HBV)感染和治疗现状。方法2014年1月1日至2014年12月31日在第三军医大学西南医院门诊和住院部就诊并检测乙型肝炎病毒血清标记物（epatitis B virus serum markers，HBV M）的重庆籍儿童（年龄≤17岁）共4 494例，对HBV M结果进行统计分析。结果 查体HBsAg、HBsAb、HBcAb阳性率及HBV M全阴性率分别为1.7%49.8%、16.6%、31.9%；1～3岁组HBsAb阳性率为65.4%，组（χ2=17.073.3、49.4，P0.05）；11～17岁组HBsAg阳性率为2.6%，组（χ2=5.612.4、8.4，P0.05）。因慢性乙型病毒性肝炎复诊儿童HBsAg确诊阳性率为89.2%，1～3岁组最低，为53.8%；-α治疗48周时血清学应答率均达到40%。在抗病毒治疗过程中，定期监测相关指标的共35例，占36.5%。结论 重庆市儿童乙型肝炎病毒查体阳性率（1.7%）已明显下降，但仍高于国家要求的防治目标（1%）；新发现HBsAg阳性的儿童应增加监测次数，需复诊明确HBsAg阳性情况。儿童抗病毒治疗并不乐观，应加强其治疗方案的管理监督。 儿童；肝炎病毒, 乙型；标记物, 血清；抗病毒药An Analysis of Children Hepatitis B virus Infection and Treatment in Chongqing Tan Shun, Tan Wenting, Xiang Xiaomei, Liu Ming, Mao Qing, Deng Guohong (Institution of Infectious Diseases, Southwest Hospital, Third Military University, Chongqing 400038, China) [Abstract] Objective To investigate the infection and treatment situation of children with hepatitis B virus in our hospital. Methods It is studied by analyzing the HBV serum markers of 4494 children from Chongqing, who were tested HBV serum markers in Southwest Hospital from January 1, 2014 to December 31, 2014. Results The positive rates of HBsAg, HBsAb, and HBcAb are 1.7%,49.8%,16.6% while the negative rate of HBV M is 31.9%. HBsAb positive rate of children aged from 1 to 3 years is 65.4%, which significantly higher than other three groups（χ2=17.0, 73.3, 49.4, P0.05）. And the HBsAg positive rate aged from 11 to 17 years is 2.6% , which significantly higher than other three groups(χ2=17.0, 73.3，49.4, P0.05). The HBsAg positive rate of chronic hepatitis B patients is 89.2%. And children aged from 1 to 3 years get the lowest rate of 53.8%. The biochemical and virological response rate are higher than 85% at 96 weeks which received antiviral treatment (LAM, LdT or ETV). And the serological response rate would reach 40% which take LAM or LdT for 96 weeks or IFN-α 4
