精品论文 参考文献 血液灌流治疗重度有机磷农药中毒疗效观察 江苏大学附属人民医院肾内科 江苏镇江 212002 摘要：目的 探讨血液灌流治疗重度有机磷农药中毒的临床疗效。方法回顾分析我院2012年1月至2015年10月收治的急性重度有机磷农药中毒42例患者，根据家庭经济能力及家属意愿分为治疗组和对照组，治疗组在使用阿托品、胆碱酯酶复能剂及对症治疗的基础上应用血液灌流治疗，对照组单纯使用阿托品、胆碱酯酶复能剂及对症支持治疗，观察比较两组患者的各项疗效指标。结果治疗组的治愈率（95.24%）高于对照组（71.43%），（Plt;0.05）；阿托品和解磷定总用量为（274.9plusmn;119.13）mg和（14.14plusmn;7.49）g，均少于对照组的（416.67plusmn;164.87）mg和（21.14plusmn;12.6）g（Plt;0.05）；胆碱酯酶恢复正常时间（8.81plusmn;3.82）d，少于对照组的（15.14plusmn;6.22）d，（Plt;0.01）；住院时间为（10.62plusmn;4.55）d，短于对照组的（18.57plusmn;8.70）d（Plt;0.05）。结论血液灌流可以明显提高有机磷农药中毒患者的治愈率，改善患者的预后，是抢救有机磷农药中毒的有效措施。 关键词：血液灌流；有机磷中毒；疗效 [Abstract]Objective To explore the clinical curative effects of hemoperfusion on severe organophosphorus pesticide poisoning.Methods 42 cases with acute severe organophosphate poisoning seen in our hospital from January 2012 to October 2015 were retrospectively reviewed.According to family economic situation and family members were divided into observation and control groups（n=21）.The control group received atropine，cholinesterase reactivator，respiratory support and so on，and the observation group also received the hemoperfusion on the basis of conventional therapy.Various clinical indicators in the two groups were observed and compared.Results The cure rate of the observation group（95.24%）was higher than that of the control group（71.43%）（Plt;0.05）.The total dosage of atropine and pralidoxime in the observation group[（274.9plusmn;119.13）mg and（14.14plusmn;7.49）g]was less than that of the control group[（416.67plusmn;164.87）mg和（21.14plusmn;12.6）g]（Plt;0.05），and its length of stay in hospital（10.62plusmn;4.55）d was shorter than that of the control group（18.57plusmn;8.70）d（Plt;0.05）.Conclusion Hemoperfusion can significanthy increase the cure rate of patients with severe organophosphorus pesticide poisoning and reduce the incidence of complications.It is an effective method for the rescue of organophosphorus pesticide poisoning.
