两种方法治疗中老年人腰椎间盘突出症临床对比研究中文摘要目的：比较双侧椎板扩大开窗椎间盘摘除椎间植骨融合内固定与单椎板开窗髓核摘 除术治疗中、老年人腰椎间盘突出症的临床疗效，寻找合理的中、老年人腰椎间盘突 出症治疗方式。方法：回顾分析 2010 年 1 月至 2010 年 6 月本科室收治 41 例的腰椎间盘突出症中、 老年患者，分别采取双侧椎板扩大开窗椎间盘摘除椎间植骨融合内固定与单侧椎板开 窗髓核摘除术治疗中、老年人腰椎间盘突出症，术后随访 6-32 个月，平均 18 个月， 定期摄片观察植骨融合和腰椎稳定性，比较两种手术方式的手术时间、术中出血、手 术费用、术后并发症、(JOA)下腰痛评分标准。运用 spss19.0 系统分析。 结果：手术时间、术中出血、手术费用有统计学意义，术后并发症、(JOA)下腰痛评 分标准无统计学意义，双侧椎板扩大开窗椎间盘摘除椎间植骨融合内固定术患者能早 起下床活动。 结论：中、老年人椎间盘突出症根据病情需要，选择适合个体化的治疗方案，均能 取得满意的临床效果。关键词中、老年人；腰椎间盘突出症；开窗；融合内固定；疗效Comparation of two treatment of senile lumbal disc herination AbstractObjective: To compare the clinic effect of bilateral vertebral lamina fenestrationdiscectomy and bone fusion and internal fixation and simple vertebral lamina fenestration discectomy on senile lumbal disc herination. To search reasonabe treatment of senile lumbal disc herination.Methods: We retrospectively analyzes the 41 cases of lumbal disc herination of elderly patients from Jan of 2010 to Jun 2010. We carry out bilateral vertebral lamina fenestration discectomy and bone fusion and internal fixation or simple vertebral lamina fenestration discectomy. The patients are fellowed-up 6-32 months, average 18 months, x-ray is taken to observe bone fusion and lumbal stability. We compare the times, blood loss, economic afford, post-operation complication and Japanese Orthopaedic Association Scores (JOA). The data is analyzed by SPSS 19.0.Results: operation times, blood loss and economic afford between the two groups appears different significantly. While post-operation complication and JOA scores appear negatively.The patients accepted bilateral vertebral lamina fenestration discectomy and bone fusion and internal fixation could leave the bed much earlier.Conclusion: Elder patients sufferring lumbal disc herination could obtain satisfying clinic effect if individual and suitable treatment is avalable.Key words: elder patients; lumbal disc herination;fenestration; bone fusion and internal fixation; clinic effect正 文两种方法治疗中老年人腰椎间盘突出
