甲状腺功能减低与老年心力衰竭关系的临床分析 　　[摘要] 目的 探析老年心力衰竭与甲状腺功能减低的相关性。方法 随机选择2011年12月―2014年12月成都平安医院心血管内科收治的317例心力衰竭老年患者作为研究对象，根据甲状腺功能分为正常组145例，亚临床甲减组105例，甲减组67例，对3组患者的心衰病程、临床终点事件发生情况、心功能分级进行对比分析。结果 甲状腺功能正常组、亚临床甲减组、甲减组患者心衰病程分别为（3.22±0.71）、（4.81±0.92）、（5.92±0.79）年；数据显示，心力衰竭病程与甲状腺功能异常具有密切的关系；甲减组患者的复合终点发生率、病死率、心力衰竭急性发作率、再住院率等分别为43.28%、13.43%、40.29%、61.19%；数据对比差异有统计学意义（P0.05）；甲减组Ⅱ级、Ⅲ级、Ⅳ级分别为32.83%、37.31%、29.85%；甲减组心功能分级最差（P0.05），主要表现在其Ⅱ级患者概率较低，而在一定程度上提高Ⅲ级、Ⅳ级的发生率。结论 老年心力衰竭与甲状腺功能具有非常密切的关系。 　　[?P键词] 心力衰竭；甲状腺功能；心功能 　　[中图分类号] R541 [文献标识码] A [文章编号] 1674-0742（2017）05（c）-0081-03 　　[Abstract] Objective To study the correlation between the hypothyroidism and senile heart failure. Methods 317 cases of senile patients with heart failure admitted and treated in our hospital from December 2011 to December 2014 were randomly selected and divided into the normal group with 145 cases， sub-clinical hypothyroidism group with 105 cases and hypothyroidism group with 67 cases， and the heart failure course， occurrence of clinical outcome events and cardiac function graduation of the three groups were analyzed and compared. Results The heart failure courses in the normal group， sub-clinical hypothyroidism group and hypothyroidism group were respectively（3.22±0.71），（4.81±0.92），（5.92±0.79）years， and the data showed that the heart failure disease had a close correlation with the abnormal thyroid function， and the incidence rate of composite end point， morbidity， acute attack rate of heart failure and re-hospitalization rate in the hypothyroidism group were respectively 43.28%，13.43%，40.29%，61.19%， and the differences was statistically significant（P0.05）， and the ratio in Ⅱlevel， Ⅲ level and Ⅳ level in the hypothyroidism group was respectively 32.83%， 37.31%， 29.85%， and the cardiac function graduation in the hypothyroidism group was the worst（P0.05）， and the main manifestation was the lower probability of patients of Ⅱ level and the incidence rate in Ⅲ level and Ⅳ level was increased to a certain degree.
