980例结肠息肉和结肠管状腺瘤应用内镜治疗临床效果观察 　　[摘要] 目的 观察结肠息肉和结肠管状腺瘤应用内镜治疗的临床效果。 方法 对我院2015年1月～2016年12月应用内镜治疗的980例结肠息肉和结肠管状腺瘤的临床效果进行观察和分析，包括息肉的直径、息肉的部位及镜下息肉的形态、病理类型及治疗前后的临床症状改善情况。 结果 980例患者共切除1216枚息肉，一次性切除无残留，平均手术时间 15～20 min，息肉切除率为 100%，镜下息肉形态分型：隆起型516 枚，平坦型700枚。病理类型：腺瘤644枚，炎性息肉347枚，增生性息肉151枚，癌变息肉74枚。治疗后980例患者的腹痛腹胀、便血、大便性状改变、腹泻、便秘、贫血、消瘦的临床表现较治疗前明显改善和缓解。 结论 内镜治疗结肠息肉和结肠管状腺瘤的临床效果好，创伤小，患者痛苦少，并发症少，值得推广和应用。 　　[关键词] 结肠息肉；结肠管状腺瘤；内镜；临床表现 　　[中图分类号] R616；R574.62 [文献标识码] B [文章编号] 1673-9701（2017）07-0092-03 　　[Abstract] Objective To observe the clinical effect of endoscopic treatment of colonic polyps and colonic tubular adenoma. Methods The clinical effect of 980 cases of colonic polyps and colorectal adenoma who were treated by endoscopy in our hospital from January 2015 to December 2016 was observed and analyzed， including the diameter of polyps， the location of polyps and polyps morphology， pathological type， as well as the improvement of the clinical symptoms before and after the treatment. Results Altogether 1216 polyps were resected in 980 patients， with one-time resection without residue. The average operation time was 15-20 min， and the polyps resection rate was 100%. Microscopic polyp morphological classification： 516 pieces of protrude type， and 700 pieces of flat type. Pathological type： 644 pieces of adenomas， 347 pieces of inflammatory polyps， 151 pieces of hyperplastic polyps， and 74 pieces of cancerous polyps. After treatment， the clinical manifestations such as abdominal pain， abdominal distension， blood in the stool， stool character changes， diarrhea， constipation， anemia， weight loss in 980 patients were significantly improved and alleviated compared with those before the treatment. Conclusion Endoscopic treatment of colonic polyps and colonic tubular adenoma has a good clinical effect， less trauma， less pain and less complications， which is worthy of promotion and application. 　　[Key words] Colonic polyps； Colonic tubular adenoma； Endoscopy； Clinical manifestation 　　结肠
