ω―3鱼油脂肪乳在肝胆胰恶性肿瘤患者术后肠外营养中应用价值 　　【摘要】 目的 研究ω-3鱼油脂肪乳在肝胆胰恶性肿瘤手术患者术后肠外营养支持中的临床价值。方法 选择52例肝胆胰恶性肿瘤切除术和完全胃肠外营养的患者，随机分为对照组和研究组。两组病人均接受等氮、等热量肠外营养支持，研究组另加用ω-3鱼油脂肪乳。比较手术前后两组病人血清总蛋白（TP）、血浆白蛋白（ALB）、前白蛋白（PA）、总胆红素（TBIL）、丙氨酸转氨酶（ALT）、天冬氨酸转氨酶（AST）、碱性磷酸酶水平（ALP）、C反应蛋白（CRP）和术后感染并发症的发生率。结果 经术后连续7天的治疗，研究组病人TBIL、ALT、AST和ALP水平明显改善（P0.05）；感染并发症亦明显少于对照组（P0.05）；CRP水平明显下降（P0.05）。结论 肝胆胰手术患者术后应用ω-3鱼油脂肪乳，有利于炎症和肝功能的恢复。 　　【关键词】 肝胆胰恶性肿瘤；ω-3鱼油脂肪乳；肝功能；肠外营养 　　doi：10.3969/j.issn.1004-7484（x）.2014.05.045 文章编号：1004-7484（2014）-05-2447-02 　　The value of ω-3 fish oil lipid Emulsion in Patients after Surgical Pperation of Hepatobiliary and Pancreatic cancer 　　WANG Zhong-zhao，YANG Fan，GU Ran 　　1.Songming County，Kunming City，Yunnan Province，Song Yang Institute of health，Yunnan Songming 651700； 　　2.Nutrition Department of the first peoples Hospital of Yunnan Province，Yunnan Kunming 650032 　　【Abstract】Objective To investigate the clinic value of ω-3 fish oil lipid emulsion in patients after surgical operation of hepatobiliary and pancreatic cancer.Methods 52 patients of post-operative hepatobiliary and pancreatic cancer were randomly divided in to control group and study group.Two groups were treated with isocaloric，isonitrogenic parenteral nutrition and the study group was added ω-3 fish oil lipid emulsion.Comparison of Serum total protein（TP），Serum albumin（ALB），Prealbumin（PA），Total bilirubin（TBIL），Alanine aminotransferase（ALT），Aspartate aminotransferase（AST），alkaline phosphates（ALP），C-reactive protein（CRP）and the rate of infection complications was made between the two groups.Results The level of TBIL，ALT，AST，ALP，CRP were improved significantly（P0.05）with reduced infection complication in the study group.Conclusion ω-3 fish oil lipid emulsion is conductive to the recovery of post-operative hepatobiliary and pancreatic cancer patients in liver function and inflammation status. 　　【Key words】 Hepatobiliary and pancreatic cancer；ω-3 fish oil lipid emulsion；Liver function；Parenteral nutrition 　　ω-3鱼油脂肪乳的主要
