中心静脉带涤纶套导管在血液透析中的应用价值 　　[摘要] 目的 探讨中心静脉带涤纶套导管在血液透析中的应用价值。 方法 通过对选自该院血透室2009年3月―2016年3月收治的46例血透患者进行研究，一组为内瘘组24例，采用经自体动静脉内瘘进行血透，另一组为导管组22例，选择因各种原因不能经自体动静脉内瘘进行血透而采用经中心静脉带涤纶套导管进行血透的患者。比较两组进行血透的血流量、并发症及尿素下降率（URR）及整体尿素清除率（KT/V），分析判断中心静脉带涤纶套导管在血透中的应用价值。结果 两组透析方法临床效果：导管组KT/V为（1.24±0.12）、URR 为（62±10.30）%，内瘘组KT/V为（1.23±0.14），URR为（63±9.70）%，（P分别为0.862、0.804，P0.05），提示两组均能有效清除体内代谢产物；导管组感染4例、栓塞2例，内瘘组感染1例、栓塞3例，（P=0.074、0.891，P0.05），两组在感染、栓塞等并发症上差异无统计学意义。而中心静脉带涤纶套导管尚有适用于外周血管条件差的患者、能即时使用、对心功能的影响小的优点。 结论 中心静脉带涤纶套导管可用于血液透析，对于不能建立内瘘和不能耐受内瘘手术的血透患者是一种理想的选择。 　　[关键词] 中心静脉带涤纶套导管；血液透析；慢性肾病 　　[中图分类号] R4 [文献标识码] A [文章编号] 1674-0742（2017）05（a）-0038-04 　　[Abstract] Objective Research into the application value of the central venous cuffed catheter in hemodialysis. Methods Select two groups of hemodialysis patients in our hospital from March 2009 to March 2016.One group of 24 cases via Cimino-Brescia arteriovenous fistula and 22 cases of central venous cuffed catheter in the other group. Compare their blood flow volume， complications， urea reduced rate（URR）and the overall KT/V and analyze the application value of the central venous cuffed catheter in hemodialysis. Results The group via central venous cuffed catheter：KT/V（1.24±0.12）； URR（62±10.30）%.The group via Cimino-Brescia arteriovenous fistula：KT/V（1.23±0.14）； URR （63±9.70）%；（P=0.862、0.804，P0.05）. Both hemodialysis ways are effective in clearing the metabolites. There are 4 cases of infection and 2 cases of embolization in the central venous cuffed catheter group and 1 case of infection and 3 cases of embolization in the Cimino-Brescia arteriovenous fistula（P=0.074， 0.891，P0.05）.There is no remarkable distinction in infection， embolization and other complications. But the central venous cuffed catheter can be applied to the patients without favorable peripheral vascular conditions.
