前路选择性手术治疗无骨折脱位颈脊髓中央损伤综合征疗效观察 　　【摘要】 目的：探?分析前路选择性手术治疗无骨折脱位的颈脊髓中央损伤综合征的临床效果。方法：选取2013年11月-2016年11月在本院接受治疗的无骨折脱位的颈脊髓中央损伤综合征患者50例作为研究对象，按照治疗方法的不同将其分为前路手术治疗组和非手术治疗组，各25例。非手术治疗组给予常规颈椎外固定激素抗感染、甘露醇脱水及营养神经等治疗，前路手术治疗组采用前路选择性手术治疗（ACDF、ACCF或其组合）。治疗结束后，对比分析两组患者手术前后的JOA评分、神经功能评分及并发症发生率。结果：手术前，两组患者的JOA评分比较，差异无统计学意义（P0.05）；手术后3、6周，前路手术治疗组的JOA评分均优于非手术治疗组，差异均有统计学意义（P0.05）。前路手术治疗组的并发症总发生率为8.0%，低于非手术治疗组的32.0%，差异有统计学意义（P0.05）。结论：前路选择性手术治疗无骨折脱位的颈脊髓中央损伤综合征有显著的临床效果，并发症较少，可有效减轻患者的疼痛，改善神经功能状况，值得临床推广应用。 　　【关键词】 前路手术； 无骨折脱位； 颈脊髓中央损伤综合征； 临床疗效 　　Clinical Effect Observation of Selective Anterior Surgical Treatment of Central Cervical Spinal Cord Injury Syndrome without Fracture and Dislocation/ZHOU Ren-shi，HUANG Hai-xiang，LIU Bing-ke，et al.//Medical Innovation of China，2017，14（09）：011-014 　　【Abstract】 Objective：To investigate the clinical efficacy of selective anterior surgical in treatment of cervical spinal cord injury syndrome without fracture and dislocation.Method： 50 cases without fracture and dislocation of cervical central cord syndrome in our hospital from November 2013 to November 2016 were selected as the research objects，according to the different methods of treatment，they were divided into the anterior surgery group and non-surgical treatment group，25 cases in each group.The non-surgical treatment group was given routine cervical fixation，anti-inflammatory hormone mannitol and nerve nutrition therapy，the anterior surgery group was treated with selective anterior surgical treatment（ACDF，ACCF or their combination），after the end of treatment， the JOA score before and after surgery，the incidence of neurological function score and complications of the two groups were compared and analyzed.Result：Before the surgery，the JOA score of two groups were compared，the difference was not statistically significant（P0.05）；3 weeks and 6 weeks after surgery，the JOA scores of anterior surgery group were better than those in non-surgical treatment group，the differences were statistically significant
