唇珠重建术探讨 　　[摘要]目的:探讨一种简便的唇珠重建方法。方法:在上唇口内近边缘处作倒V形切开,应用V-Y改形原理,将皮瓣往唇缘处推进,加上唇系带二侧旋转瓣相嵌缝合,重建唇珠。结果:皮瓣血运、存活良好,重建唇珠外形满意,表面看不到瘢痕,是一种简单、方便、理想的唇珠重建方法。随访3～15个月,平均12个月,皮瓣质地优良,瘢痕不明显,无挛缩,唇形生动自然,立体感强。远期效果满意。结论:运用V-Y改形术加上唇系带二侧旋转瓣相嵌缝合术是进行唇珠重建的理想术式。手术设计及操作简便,皮瓣血运可靠,能理想地重建缺损的唇珠,术后不挛缩,瘢痕不明显,远期效果可靠。 　　[关键词]上唇系带;V-Y改形;唇珠重建 　　[中图分类号]R782 [文献标识码]A [文章编号]1008-6455(2010)02-0195-03 　　 　　Searching for reconstruction of the central vermillion tubercle 　　WANG Yong-ding,SHAO Wen-hui,PU Xing-wang,CHEN Yuan-kun,WANG Ying-ying,XIAO Qing-chang 　　(Sichuan Xichan Aesthetic Plastic Hospital,Chengdu 610041,Sichuan,China) 　　 　　Abstract: ObjectiveSearching for a new surgical technique in reconstruction of the central vermillion tubercle.MethodsAfter making a V-shaped incision on the oral-wet vermillion-mucosal aspect of the upper lip,we lift the flap and extend the frenulum,coupled with phase filleting of the two sides of labial frenums rotation flap,the lip tubercle then could be obtained by V-to-Y closure of the incision.ResultsThe flaps had agood blood circulation and the patient showed a natural contour of the lip.This new proceture is an easy technique of the reconstruction of the lip tubercle.The average follow-up was 1 year. the flap was in good condition,the scarring was inconspicious,the results were found to be longlasting at later term. Conclusion It is the ideal surgical reconstruction of the central vermillion tubercle to reshape V-to-Y coupled with phase filleting of the two sides of labial frenums rotation flap.The V-to-Y plasty plus the extention of the frenulum is a very easygoing and adjustable technique to reshape the lip tubercle. This new procedure is easy to perform and also has a good survival of the graft,could be an ideal technique in reconstruction of the central vermillion tubercle.There was no obvious scarring nor granuloma formation, the long-term results are reliable. 　　Key words:frenulum of upper lip reconstruction of the central vermillion tubercle V-to-Y closure
