社区分层护理模式结合药物治疗老年人2型糖尿病应用研究 　　【摘要】 目的：探索社区分层护理模式结合药物治疗老年人2型糖尿病的应用效果。方法：选取中山市某镇的460例老年2型糖尿病患者为试验组，进行1年的社区分层护理模式结合药物治疗，选取另外一个镇的510例老年患者为对照组进行常规干预，比较研究前后患者的血糖控制、卫生行为和生活质量的变化情况。结果：经过1年的干预，两组的空腹血糖、餐后血糖和糖化血红蛋白水平均较干预前下降，差异均有统计学差异（P0.05）。结论：社区分层护理模式结合药物治疗老年人2型糖尿病的效果满意，值得在社区中推广。 　　【关键词】 糖尿病； 社区； 分层护理模式； 老年人 　　The Effects of Stratified Nursing Model Combined with Drug Therapy for Elderly Community Type 2 Diabetes/LIANG Pei-shao，ZHENG Zhi-hong，WANG Cai-xia.//Medical Innovation of China，2015，12（08）：071-074 　　【Abstract】 Objective：To evaluate the effect of stratified nursing model combined with drug therapy for community edlerly type 2 diabetes.Method：460 edlerly type 2 diabetes patients were came from a town as experimental group and received stratified nursing model combined with drug therapy intervention. The other 510 edlerly diabetes patients were came from other communities as control group and received conventional therapy. The blood glucose， blood fat， health behaviors and living quality were compared between the two groups. Result：After one-year intervention， the levels of fasting glucose， 2-hour postprandial blood glucose， and HbAlc in the two groups were significantly reduced （P0.05）. And those in the experimental group were significantly lower than those in the control group（P0.05）. The health behaviors （self-test blood glucose， diet control， regular exercise， weight control， taking medicine regularly）， the score of living quality （healthy sense， physical symptoms， ability to work， cognitive function， ability to community participation） of the patients form the experimental group were significantly improved after one-year intervention（P0.05）. And the rules of the control group after the intervention rate improved， physical symptoms and cognitive function， the rest on improvement were not obvious （P0.05）.Conclusion：Stratified nursing model combined with drug therapy for community edlerly type 2 diabetes is effective. It’s worthy of further application and promotion
