瘢痕子宫剖宫产术中出血原因分析及预防措施探讨 　　【摘要】 目的：探讨瘢痕子宫剖宫产术中出血的原因及预防措施。方法：231例瘢痕子宫剖宫产术术中出血产妇为观察组，234例首次剖宫产术产术中出血产妇为对照组，采取常规子宫下段剖宫产手术生产，对比两组产妇术中出血量、术中出血发生率及出血因素。结果：观察组术中出血量（186.23±36.28）ml、术中出血量发生率为7.14%，高于对照组的（159.48±28.45）ml、1.71%，差异有统计学意义（P0.05）。结论：瘢痕子宫剖宫产较首次剖宫产更容易出现血术中出血，主要原因包括胎盘因素、宫缩乏力等，应该实施相应的措施降低瘢痕子宫剖宫产术中出血率。 　　【关键词】 瘢痕子宫； 剖宫产； 术中出血； 原因 　　doi：10.14033/j.cnki.cfmr.2017.28.059 文献标识码 B 文章编号 1674-6805（2017）28-0113-02 　　【Abstract】 Objective：To investigate the causes and prevention of bleeding during cesarean section.Method：231 cases of scar uterus cesarean section bleeding maternal as the observation group，234 cases of the first cesarean section from bleeding women as the control group，with routine uterine cesarean section surgery.Maternal blood loss，intraoperative bleeding and bleeding factors were compared between the two groups.Result：The amount of the observation group with intraoperative bleeding （186.23±36.28）ml，the recurrence rate of 7.14% were higher than the control group of the （159.48±28.45）ml，1.71%，the differences were statistically significant（P0.05）.Conclusion：The scar uterus cesarean section compared with cesarean section for the first time more easily bleeding during the operation，the main reasons include placental factors，uterine atony，should implement the corresponding measures to reduce the bleeding rate of scar uterus during cesarean section. 　　【Key words】 Scar uterus； Cesarean section； Intraoperative hemorrhage； Cause 　　First-author’s address：The People’s Hospital of Luzhi in Wuzhong District of Suzhou City，Suzhou 215127，China 　　?S着医学技术的发展，剖宫产安全性得到很大的提升，同时由于社会理念的变化，选择剖宫产的孕妇越来越多，瘢痕子宫再次妊娠也随之增加，瘢痕子宫剖宫产比例上升尤为明显[1]，剖宫产可提高难产和身体状况较差产妇分娩的成功率，但剖宫产产妇很容易出现术中出血，对母婴健康与生命安全构成严重威胁[2]。本文对比观察首次剖宫产术和瘢痕子宫剖宫产术的出血情况，分析其出血因素，制定相应对策，以期减少术中出血量，降低出血率，现报道如下。 　　1 资料与方法 　　1.1 一般资料 　　选取江苏省苏州市吴中区?f直人民医院妇产科2015年 　　1月-2016年12月实施剖宫产的孕产妇作为研究对象，其中231例瘢痕子宫剖宫产术术中出血产妇为观察组，年龄22～40岁，平均（28.93±5.12）岁，孕周34～41周，平均（37.54±1.34）周；234例首次剖宫产术产术中出血产妇为对照组，年龄22～39岁，平均（28.03±4.97）岁，孕周32～41周，平均（37.68±
