研究自体血回输在外伤性脾破裂手术中临床应用价值 　　[摘要] 目的 分析自体血回输在外伤性脾破裂手术中的临床应用价值。方法 方便选取2012年2月―2017年2月收治的50例外伤性脾破裂手术患者，在其手术过程中对其进行自体血回输治疗，研究其治疗效果。结果 50例患者在手术过程中的总失血量为534 500 mL，自体回输血量为32 600 mL，平均自体回输血量为720 mL，最多自体回输血量为1 700 mL，最少自体回输血量为400 mL；手术前，患者APTT为（35.42±3.56）s，手术后为（36.01±4.12）s，对比差异无统计学意义（t=0.766 2，P=0.445 4）。手术前，患者PT为（12.45±0.81）s，手术后为（12.58±1.02）s，对比差异无统计学意义（t=0.705 8，P=0.482 0）。结论 自体血回输在外伤性脾破裂手术中的临床应用价值显著，值得推广。 　　[关键词] 自体血回输；外伤性脾破裂；临床应用价值 　　[中图分类号] R65 [文献标识码] A [文章编号] 1674-0742（2017）11（c）-0075-03 　　[Abstract] Objective This paper tries to analyze the clinical value of the transfusion rupture surgery in traumatic splenic autologous blood. Methods 50 cases of traumatic spleen rupture surgery from February 2012 to February 2017 were conveniently selected， adopting the autologous blood transfusion treatment in the operation process， and its therapeutic effect was studied. Results The total blood loss in 50 patients during surgery was 534 500 mL， autologous transfusion volume was 32 600 mL， average autotransfusion of blood was 720 mL， the most autotransfusion of blood was 1 700 mL， the minimum of autotransfusion of blood of 400 mL patients； before the surgery， APTT was （35.42±3.56）s， after surgery for （36.01±4.12）s， with no significant difference （t=0.766 2， P=0.445 4）. Before surgery， patients with PT of （12.45±0.81）s， after the operation of（12.58±1.02） s. There was no significant difference between the two groups （t=0.705 8， P=0.482 0）. Conclusion Autologous blood transfusion in the treatment of traumatic spleen rupture in operation has significant clinical application value， so it is worthy of promotion. 　　[Key words] Autologous blood transfusion； Traumatic rupture of spleen； Clinical application value 　　在外科中，外??性脾破裂属于一种常见的急腹症，其出血具有急、猛的特点，如果不及时采取有效措施对其进行处理，患者在短时间内便会休克，严重的情况下甚至会促使患者死亡[1]。对患者进行成功救治的关键在于及时进行手术止血，并且对其进行血容量补充。手术过程中，对患者进行救治的一项重要措施为术中输血，但是在输血过程中，通常会遇到血源性传染病感染与血源不足等问题，为了对输血引发的一系列不良问题进行有效避免，国内多数学者主张采用自体输血的方式[2]。随着医学技术水平的不断提高，自体血回输在外科手术中得到了广泛应用，该技术不仅能够将血源缺乏的问题有效解
