社区居家养老模式现状及其对老年慢性病管理作用和影响 　　[摘要]目的 调查研究社区居家养老模式现状及其对老年慢性病管理的作用和影响。方法 于2016年7～12月，在江西、宁夏、上海、贵州、新疆等地区选取部分市县的社区，采用问卷调查的方法对老人进行有关个人健康状况，社区居家养老模式现状及慢性病管理情况的调查。结果 老年人慢性病患病率较高，心脑血管疾病（68.1%），糖尿病（40.3%），骨质疏松（28.7%）为老年人群中发病率最高的三大疾病。社区居家养老模式在我国较为普遍，其为老年人提供了体检、健康咨询、静脉补液、看病治疗等服务。社区居家养老模式仍存在服务模式单一、医疗卫生资源不足等问题，但其服务能够增强老年人对慢性病的意识，延缓病情的发展，降低医疗费用。结论 老年人的健康状况较不理想，慢性病患病率较高。社区居家养老模式存在不足，但其提供的多项服务对于老年慢性病的管理与防治具有一定的积极意义，能够降低养老成本，提高老年人生活质量。 　　[关键词]慢性病管理；老年人；社区居家养老模式 　　[中图分类号] R473.2 [文献标识码] A [文章编号] 1674-4721（2017）05（b）-0128-04 　　The status of community home care model，and its role and impact on management of chronic diseases in the elderly 　　ZHU Fei-ying PAN Gui-xing MIN Ya-ting WANG Xiao GONG Zhi-wu ZHONG Qing-ling 　　Medical College of Nanchang University in Jiangxi Province，Nanchang 330031，China 　　[Abstract]Objective To study the status of community home care model，and its role and impact on management of chronic diseases in the elderly.Methods From July 2016 to December 2016，some of the communities were selected from party of Jiangxi，Ningxia，Shanghai，Guizhou，Xinjiang and other cities.The questionnaire was used to investigate the health status of the elderly，current situation of community home care model and management of chronic diseases.Results The prevalence rate of chronic diseases was high in elderly patients，cardiovascular and cerebrovascular diseases （68.1%），diabetes mellitus （40.3%），and osteoporosis （28.7%） were the highest incidence of the three major diseases in the elderly population.Community home care model was common in China，which provides physical examination，health counseling，intravenous fluids，medical treatment and other services for the elderly.There are still some problems such as single service mode，insufficient health resources.However，this service could enhance the elderly′s awareness of chronic diseases，delayed the development of the disease and reduced the cost of medical treatment.Conclusion The health status of the elderly is not satisfactory，and the p
