社区开展药物重整工作实践效果分析 　　摘 要 目的：对社区老年慢性病患者进行药物重整并进行效果分析。方法：选取2015年7―9月在中心就诊的老年高血压合并糖尿病患者211例，其中男性86例（40.76%），女性125人（59.24%）。年?g范围62～95岁，平均（74.65±8.16）岁。在12个月内对患者进行药物重整干预6次，干预前后进行问卷调查评估药物重整指导效果。结果：干预后长期用药品种显著减少，每月药费总金额和每月药费自付金额显著降低（P均0.01），一般健康状况和精神健康评分显著提高（P0.05），患者满意度和患者健康自评分明显上升（P均0.01）。结论：药物重整可降低用药费用，提高老年慢性病患者生活质量。 　　关键词 慢性病；药物重整；社区 　　中图分类号：R95 文献标志码：A 文章编号：1006-1533（2017）14-0048-02 　　Analysis of the effect of development of medication reconciliation in the community 　　YANG Nan 　　（Zhenru Community Health Service Center of Putuo District， Shanghai 200333， China） 　　ABSTRACT Objective： To carry out medication reconciliation on elderly patients with chronic diseases in the community and analyze the effect. Methods： A total of 211 cases of hypertension and diabetes mellitus from July to September 2015 were selected in the center for treatment， among them 86 cases were male （40.76%） and 125 female （59.24%）. The age range was 62～95 years old and average （74.65±8.16） years. The medication reconciliation intervention was carried out six times for elderly patients with chronic diseases within 12 months. A questionnaire survey was conducted before and after intervention to evaluate the effect of medication reconciliation. Results： After the intervention， the long-term medication varieties decreased significantly， and the total amount of monthly expenses for medicine and the amount of monthly out-of-pocket payment for medicine were significantly reduced（all P0.01）. The general health status and mental health score were significantly improved（P0.05）. The scores of patient satisfaction and patient health self-scoring increased obviously（P0.01）.
