超声检查对甲状腺微小乳头状癌诊断价值 　　 　　[摘要] 目的 探讨术前超声检查对甲状腺微小乳头状癌（PTMC）的诊断价值。 方法 选取本院2013年1月~2013年12月收治的PTMC患者69例经手术和病理证实的共78个PTMC病灶超声图像，与同期随机选取的50个良性结节声像图进行对照。 结果 69例PTMC患者与病理诊断相符的有56例，相符率为81.16%（56/69），误诊率为10.14%（7/69），漏诊率为8.70%（6/69）；78个PTMC结节的直径超声检测值为（10.56±8.35）mm，病理镜检测值为（3.95±1.23）mm，超声检测值与病理镜检测值比较，差异有统计学意义（t=5.551，P=0.000）；78个超声诊断的PTMC结节在形态、边界、纵横比＞1等方面与50个良性节比较，差异有统计学意义（P0.05）。 结论 超声检查对PTMC虽然具有一定的缺陷，但其临床应用价值仍然较大。 　　[关键词] 甲状腺微小乳头状癌；超声检查；诊断价值 　　[中图分类号] R730.41[文献标识码] A[文章编号] 1674-4721（2014）03（c）-0114-03 　　 　　The diagnostic value of ultrasound examination in papillary thyroid microcarcinoma 　　ZHANG Hao1 YIN Xi-yu1 LI Xiao-hui2 　　1.Department of Ultrasound,Nanwan People′s Hospital of Longgang District in Shenzhen City,Shenzhen 518114,China;2.Department of Function,Zhonghai Hospital of Shenzhen City,Shenzhen 518000,China 　　[Abstract] Objective To explore the diagnostic value of preoperative ultrasound examination for papillary thyroid microcarcinoma (PTMC). Methods Ultrasound image of 78 lesions confirmed by operation and pathology of PTMC in 69 cases with PTMC from January to December 2013 in our hospital were selected,and they were compared with the sonogram 50 benign nodules selected randomly. Results Among 69 patients with PTMC,there were 56 cases matching with the pathological diagnosis,the coincidence rate was 81.16% (56/69),the misdiagnosis rate was 10.14% (7/69),the missed diagnosis rate was 8.70% (6/69);diameter of 78 PTMC nodule in ultrasonic testing was (10.56 +8.35)mm,the value of pathological mirror detection was (3.95+1.23)mm,and there was a statistical difference of detection value between the two groups (t=5.551，P=0.000);78 ultrasonic diagnosis of PTMC nodules in shape,boundaries,the aspect ratio (more than 1) etc were significantly different with the 50 benign nodules (P0.05).
