山西医科大学硕士论文 山西医科大学硕士论文 II II 灶进行随访检查，对比分析其对病灶局部进展检测的灵敏度、特异度等相关统 计学指标。结果 随访 3-12 月，829 个病灶微波消融治疗后 766 个病灶完全灭活， 63 个病灶局部进展。两种检测方法的灵敏度、特异度、误诊率、漏诊率分别为 92.06% VS 93.65%； 99.74% VS 99.21%；0.26% VS 0.13%；7.94% VS 6.35%。 超声造影与磁共振成像对肝癌微波消融后肿瘤局部进展的检测无明显差异(校正 x2=0.125 p值=0.7380.05)。结论 超声造影与磁共振成像对肝癌消融术后肿瘤 局部进展的检测有相似的灵敏度和特异度，超声造影可作为检测肝癌消融术后 是否存在肿瘤局部进展的一种有效方法。 关键词：超声造影 磁共振 局部进展 III III The current clinical efficacy of contrast-enhanced ultrasound-guided percutaneous microwave ablation for liver cancer lesions PART ONE The clinical efficacy study of contrast-enhanced ultrasound-guided microwave ablation for liver cancer which showed unclear image under conventional ultrasound Abstract Object To explore the superiority and feasibility of the microwave ablation guided by contrast-enhanced ultrasound(CEUS) on treating liver cancer which showed unclear image under conventional ultrasound. Methods From January 2011 to September 2011, the clinical data of 172 patients with liver cancer were collected in our hospital. There were totally 182 lesions which showed unclear image under conventional ultrasound but could be found under contrast-enhanced ultrasound, There were 148 male patients and 34 female patients with a mean age 56.4±10.8 years. The maximum diameter of the tumors was 46mm while the minimum was 5mm. The average diameter was 12.5±8.9mm. The CEUS was performed before the microwave ablation to search the lesions. Results After 1-2 times’ CEUS, 177 lesions were found. then microwave ablation was successfully performed under CEUS. The technical success rate was 97.3% (177/182). The follow-up duration was 3-12 months (mean =6.7 months). The local tumor progression rate was 3.4% (6/177), There were no serious complications were observed in patients. Conclusion CEUS-guided microwave ablation is a reliable, feasible and therapy for patients whose hepatic were unclear under conventional ultrasound.
