Background/Recitals
WHEREAS,theAuthorizerhasundergonecertainmedicalexaminationsandgeneratedspecificmedicalimaginginformation;and
WHEREAS,theAuthorizerwishestoauthorizetheAuthorizedPartytousetheaforementionedmedicalimaginginformationforaspecificpurpose;and
WHEREAS,theAuthorizedPartyisqualifiedanddesirestoreceiveandusethemedicalimaginginformationunderthetermsandconditionssetforthherein.
SubjectMatter
1.Themedicalimaginginformationauthorizedherebyrefersspecificallytothedigitalmedicalimagesandassociatedmetadatapertainingtotheexamination(s)conductedon[PatientFullName]at[OriginalHealthcareInstitutionName]onoraround[Date(s)ofExamination],includingbutnotlimitedto[Listspecificimagingmodalities,e.g.,ChestX-ray,AbdominalCT],identifiedbyImageID(s)[ListImageIDsifknown],andanyrelateddiagnosticreportsgeneratedonoraround[Date(s)ofReportGeneration].Thescopeincludestherawimagedataandanymetadatalinkedtothepatientrecordfortheseexaminations.
2.Themedicalimaginginformationmaybeindigitalformat(e.g.,DICOMfiles)orotherforms(e.g.,printedfilms)asoriginallycreatedorprovidedbytheOriginalHealthcareInstitution.
