X医院肿瘤科PICC带管出院患者延续性护理需求的质性研究

【摘要】目的：?探讨X医院肿瘤科PICC带管出院患者对延续性护理的具体需求，为制定针对性干预措施提供依据。方法：?采用描述性质性研究设计，通过目的抽样选取84例PICC带管出院肿瘤患者进行半结构式访谈，运用Colaizzi现象学分析法对资料进行编码和主题提炼。结果：?提炼出四大需求主题：导管护理知识需求（85.7%患者关注日常观察与并发症识别）、心理支持需求（70.2%存在导管安全焦虑）、医疗延续服务需求（90.5%期望上门护理）、信息获取渠道需求（75.0%倾向图文/视频指导）。不同年龄、文化程度及疾病类型患者的需求存在显著差异（P0.05）。患者对现有出院宣教满意度仅33.3%，但对建立咨询热线（90.5%）及视频教程（81.0%）需求强烈。结论：?PICC带管出院患者存在多维度、未满足的延续性护理需求，需构建系统化、个性化及信息化支持的延续护理模式，以提升居家护理质量与患者安全。

Abstract:Objective:ToexplorethespecificneedsofpatientswithPICCcathetersdischargedfromtheoncologydepartmentofXHospitalforcontinuouscare,andtoprovideabasisforformulatingtargetedinterventionmeasures.Methods:Adescriptivequalitativeresearchdesignwasadopted.Eighty-fouroncologypatientswithPICCcathetersatdischargewereselectedthroughpurposivesamplingforsemi-structuredinterviews.Colaizzisphenomenologicalanalysismethodwasusedtocodeandextractthemesfromthedata.Results:Fourmajordemandthemeswereextracted:knowledgeneedsforcathetercare(85.7%ofpatientswereconcernedaboutdailyobservationandcomplicationidentification),psychologicalsupportneeds(70.2%hadanxietyaboutcathetersafety),medicalcontinuationserviceneeds(90.5%expectedhomecare),andinformationacquisitionchannelneeds(75.0%preferredguidancethroughpictures/videos).Thereweresignificantdifferencesintheneedsofpatientsofdifferentages,educationallevels,anddiseasetypes(P0.05).Thesatisfactionrateofexistingdischargeeducationwasonly33.3%,buttherewasastrongdemandforestablishingaconsultationhotline(90.5%)andvideotutorials(81.0%).Conclusion:PatientswithPICCcathetersdischargedfromthehospitalhavemulti-dimensionalandunmetneedsforcontinuouscare.Asystematic,personalized,andinformation-basedcontinuouscaremodelneedstobeconstructedtoimprovethequalityofhomecareandpatientsafet
