左旋氨氯地平对血压正常冠心病患者血管内皮功能的影响 摘要：目的　探讨左旋氨氯地平对血压正常冠心病患者内皮功能的影响。　方法　选择经血管造影确诊的100例血压正常的稳定性CAD患者，随机分为常规组50例和玄宁组(与左旋氨氯地平的关系应明确化说明)50例，观察疗程共18个月。运用高分辨超声及实验室方法，观察FMD、IMT、ET1及CRP等各项试验指标变化情况，来观察玄宁对大动脉内皮功能和动脉粥样硬化指标的影响。结果　两组患者治疗前、后血压均无显著性差异(Ｐ0.05)； FMD、IMT、ET1及CRP等参数治疗前后相比，玄宁组明显优于常规组（P0.05）。与治疗前相比有显著性差异(Ｐ0.01) (你所要表达的是同组在治疗前后相比，治疗组与常规组之间同期相比。而这三句话是并没有表达清楚！)。　结论　左旋氨氯地平改善血管内皮功能，延缓动脉粥样(硬化)方面的作用并不完全依赖于血压的降低，且应用于血压正常的CAD患者仍是安全的(结论有点大了)。 关键词：　氨氯地平;　冠心病;　颈动脉内膜中层厚度;　内皮功能 Influence of L-Amlodipine on the Endothelial Function of CHD Patients with Normal BP and Carotid Arterial Intima-media Thickness LI Hai-bin，XIAO Wen-liang,JIANG Zhi-an,ZHANG Xiao-guang,SU Rui-ying,ZHAO Shu-jian (Department of Cardiology,the Third Hospital Affiliated to Hebei Medical University,ShiJiazhuang,Heibei) ABSTRACT：Obective To investigate the influence of L-amlodipine on the endothelial function of CHD patients with normal BP and carotid arterial intima-media thickness.Method 100 subjects with normal BP and angiographically confirmed stable CHD were enrolled into the present study and randomly divided into two groups,xuanning group(n=50) and control group(n=50).The observation lasted 18 months. Serial measurements of FMD IMT and blood levels of CRP and ET-1 were performed with high-resolution ultrasound and laboratory methods to assess the effects of xuanning on endothelial function and atherosclerosis of large arteries.Result There were no significance in blood pressure between two groups through treatments(P0.05).Compared with patients in control group, FMD IMT and serum levels of CRP ET-1 in xuanning group were much better reserved and improved(P0.05),and differ significantly compared with them before treatments(P0.01).Conclusion The effects that L-amlodipine improve vascular endothelial function and delay the progression of atheroclerosis did not totally depend on the fall of BP level,and it’s also safe to use L-amlodipine in treatment of CAD patients with normal BP.
