临床医学论文-经皮椎体成形术治疗骨质疏松性椎体压缩性骨折 ?????????? 作者：梅盛前，陈其昕，徐利明，兰俊? 摘要:目的? 探讨经皮椎体成形术治疗骨质疏松性椎体压缩骨折的临床效果。 方法? 自2004年1月至2004年9月采用经皮椎体成形术治疗骨质疏松性椎体压缩骨折患者21例，与同期保守治疗的19例患者进行对比研究。结果? 经皮椎体成形术组与保守治疗组相比，住院时间明显缩短，患者症状缓解较早，早期可开始下床活动（P0.01），所有病例伤椎前缘高度恢复明显，经统计学分析差异有显著性（P0.05），而后凸Cobb角矫正不明显，差异无统计学意义（P0.05），随访6～12个月（平均8个月）后未见明显伤椎高度丢失。 结论? 经皮椎体成形术是一种简单、微创、安全有效的治疗方法，但要掌握其应用的适应证。???? 　　关键词: 经皮椎体成形术;椎体压缩骨折;骨质疏松;治疗 　　Treatment of Osteoporotic Vertebral Compression Fractures with Percutaneous Vertebroplasty????? 　　Abstract:Objective To investigate the clinical outcomes in the treatment of Osteoporotic Vertebral Compres-sion Fractures with Percutaneous Vertebroplasty（PVP）.Methods The clinical results of a group of21patients，who had suffered from the Osteoporotic Vertebral Compression Fractures and treated with Percutaneous Vertebro-plasty，were compared Fractures and treated with Percutaneous Vertebroplasty，were compared with another group of19patients treated by non-operative procedure.Results The duration of hospitalization，the time of pain remis-sion and the out of bed activity were significantly reduced in the PVP.group as compared with those undergone non-operative procedure（P0.01）.On following up from six months to tweleve months，the anterior height of injured vertebral body of all patients were generally recovered obviously（P0.05），and the Cobb angles were not rectified obviously（P0.05）.Conclusion The PVP is an effective procedure of simple microinjure，with less complication，wide indication and good outcome，and it is essential to master its indication meanwhile.??? 　　Key words:percutaneous vertebroplasty;vertebral compression fracture;osteoporotic;treatment 　　骨质疏松症的患者日益增多，而脊柱压缩性骨折是骨质疏松症的常见并发症。多数骨折会在几周或几个月内愈合，但部分患者会留有持续性腰背疼痛症状，严重影响了患者的生活质量。经皮椎体成形术（percutaneous vertebroplasty，PVP）是经皮穿刺通过椎弓根向椎体内注入骨水泥，以达到恢复椎体高度，增强椎体强度，防止塌陷，缓解腰背部疼痛的目的。我院自2004年1月至2004年9月采用经皮椎体成形术治疗骨质疏松性椎体压缩骨折患者21例，与同期保守治疗的19例患者进行对比研究，结果充分肯定了经皮椎体成形术的优越性，现报告如下。 　　1 材料与方法 　　1.1 一般资料??? 　　1.1.1 椎体成形组 本组共21例，其中男7例，女14例;年龄61～
