Decreasedβ-amyloid andincreasedphospho-tauproteinlevelsinthecerebrospinal?uid(CSF)arecurrentlythemostaccurate 1?42 chemicalneurodiagnosticsofsporadicAlzheimerdisease(AD).Areport(2007)oftheThirdCanadianConsensusConferenceon theDiagnosisandTreatmentofDementia(2006)recommendedthatbiologicalmarkersshouldnotbecurrentlyrequisitionedby primarycarephysiciansintheroutineinvestigationofsubjectswithmemorycomplaints.Considerationforsuchtestingshould promptpatientreferraltoaspecialistengagedindementiaevaluationsoraMemoryClinic.Thespecialistshouldconsiderhaving CSFbiomarkers(β-amyloid andphospho-tau)measuredatareputablefacilityinrestrictedcasespresentingwithatypical 1?42 featuresanddiagnosticconfusion,butnotasaroutineprocedureinallindividualswithtypicalsporadicADphenotypes.We submitthatdevelopmentsinthe?eldofADbiomarkerdiscoverysincepublicationofthe3rdCCCDTDconsensusdatadonot warrantrevisionofthe2007recommendations. 1.Introduction perspectiveontheclinicaluseofADbiomarkersinCanada basedontheThirdCanadianConsensusConferenceonthe DiagnosisandTreatmentofDementia[1].
