Indication.Theexcisionvolumewas20%to40%ofthebreast volume,andthebreastdensitywasdense.SurgicalTechnique.Theupperedgeofthebreastatthesubclavicularareawasdrawnin thestandingpositionbeforesurgery.Afterpartialmastectomy,anextendedglandular?apwasmadebyfreeingthebreastfromboth theskinandthepectoralisfasciauptothepreoperativemarkinginthesubclaviculararea.Itisimportanttokeeptheperforators oftheinternalmammaryarteryand/orthebranchesofthelateralthoracicarteryintactwhilemakingthe?ap.Results .Seventeen patientsunderwentremodelingusinganextendedglandular?ap.Thecosmeticresultsat1yearaftertheoperation:excellentin11, goodin1,fairin3,andpoorin2.Allcasesofunacceptableoutcomeexceptonewerecaseswithcomplications,andmorethan30% resectionofmoderateorlargesizebreastsdidnotobtainanexcellentresultforlong-termfollowup.Conclusion.Thistechniqueis usefulforperformingthebreast-conservingreconstructionofsmalldensebreasts. 1.Introduction breastcanbemobilizedwithunderminingandadvancement ofbreasttissuewithoutcomplications.However,low-density breasttissuewithmajorfattycomponentshasamuchhigh
