Beyond the neonatal period, S. agalactiae is rarely found in the respiratory tract. During 2002–2008 we noticed S. agalactiae in respiratory secretionsof30/185(16%)ofcysticfibrosis(CF)patients.Themedianageofthesepatientswas3–6yearsolderthanthe medianageCFpatientsnotharboringS.agalactiae.Toanalyze,iftheS.agalactiaeisolatesfromCFpatientswereclonal, furthercharacterizationofthestrainswasachievedbycapsularserotyping,surfaceproteindeterminationandmultilocus sequencetyping(MLST).Wefoundavarietyofsequencetypes(ST)amongtheisolates,whichdidnotsubstantiallydiffer fromtheMLSTpatternsofcolonizingstrainsfromGermany.HoweverserotypeIII,whichisoftenseenincolonizingstrains andinvasiveinfectionswasrareamongCFpatients.TheemergenceofS.agalactiaeintherespiratorytractofCFpatients mayrepresenttheadaptationtoanovelhostenvironment,supportedbythealteredsurfactantcompositioninolderCF patients.
