We investigated A. baumannii isolates at a New York hospital to characterizegeneticrelatedness. Methods:ThirtyA.baumanniiisolatesfromgeographically-dispersednursingunitswithinthehospitalwerestudied.Isolate relatednesswasassessedbyrepetitivesequencepolymerasechainreaction(rep-PCR).Thepresenceandcharacteristicsof integronswereassessedbyPCR.Metabolomicprofilesofasubsetofaprevalentstrainisolatesandsporadicisolateswere characterizedandcompared. Results:Wedetectedahospital-widegroupofcloselyrelatedcarbapenemresistantMDRA.baumanniiisolates.Compared withsporadicisolates,theprevalentstrainisolatesweremorelikelytobeMDR(p=0.001).Isolatesfromtheprevalentstrain carriedanovelClassIintegronsequence.Metabolomicprofilesofselectedprevalentstrainisolatesandsporadicisolates weresimilar. Conclusion:TheA.baumanniipopulationatourhospitalrepresentsaprevalentstrainofrelatedMDRisolatesthatcontaina novelintegroncassette.Prevalentstrainandsporadicisolatesdidnotsegregatebymetabolomicprofiles.Furtherstudyof environmental, host, and bacterial factors associated with the persistence of prevalent endemic A. baumannii strains is neededtodevelopeffectivepr
