Methods:In-housewineswereproducedforthestudy,finedwithdifferentconcentrationsofcasein(n=7),egg albumin(n=1)andisinglass(n=3).ELISAandPCRkitsspecificfortherespectiveproteinswereusedtoidentify thefiningagents.SkinpricktestsandbasophilactivationtestswereperformedinpatientswithconfirmedIgE- mediatedrelevantfoodallergies(n=24).Awineconsumptionquestionnaireanddetailedhistoryonpossible reactionstowinewasobtainedinamultinationalcohortofmilk,eggorfishallergicpatients(n=53)andpatients allergictoirrelevantfoodsascontrols(n=13). Results:Finingagentswerenotdetectableinwineswiththeavailablelaboratorymethods.Nevertheless,positive skinpricktestreactionsandbasophilactivationtotherelevantwineswereobservedinthemajorityofpatients withallergytomilk,eggorfish,correlatingwiththeconcentrationofthefiningagent.Amongpatientsconsuming wine,reportedreactionswerefewandmildandsimilarwiththeonesreportedfromthecontrolgroup. Conclusion:Casein,isinglassoregg,remainingintracesinwineafterfining,presentaverylowriskforthe respectivefoodallergicconsumers.Physicianandpatientawarenesscampaignsmaybemoresuitablethan generalizedlabelingtoaddressthisissue,asthelattermayhavenegativeimpactonbothnon-allergicandallergic consumers.
