· 6 · 解放军护理杂志 2008年6月，25(6A) 颈动脉狭窄患者颈动脉内膜剥脱术前后血流动力学的改变 闫雅凤，侯惠如，杨 丽 (解放军总医院 南楼临床部，北京100853) 摘要：目的 观察颈动脉狭窄患者实施颈动脉内膜剥脱术手术前后血流动力学改变的规律，为临床护理提供依据。 方法 对实施颈动脉内膜剥脱术的患者，于术前7 d、3 d、1 d每日晨和术后1周进行血流动力学的监测，对术前、术 后血流动力学指标进行统计学分析。结果 患者自身术前 SBP、DBP、MBP对比，患者术后SBP、DBP、MBP对比， 手术前后心率、手术前后呼吸分别进行对比，P均o．05，无统计学意义；手术前后SBP、DBP、MBP对比、手术前后 心律对比，P均O．01，有统计学意义；并且手术后 71．0 的患者SBP、DBP、MBP呈现升高趋势，38．7 的患者术 后出现心律不规则，心律变化由大变小逐渐趋于正常。结论 颈动脉狭窄患者手术后血流动力学变化明显，手术 后应注重血流动力学的监测，以预防并发症的发生。 关键词：颈动脉狭窄；内膜剥脱术；血流动力学；监测 中图分类号：R473．6 文献标志码：A 文章编号：1008—9993(2008)6A一0006—03 Changes of Hemodynamics in Patients with Carotid Stenosis before and after Carotid En— doarterectomy YAN Ya-feng，HOU H ui—ru，YANG I i(Department of Clinical Medicine，South Building，General Hospital of PLA，Beijing 100853，China) Abstract：Objective To observe the hemodynamics changes in carotid stenosis patients before and after carotid endarterectomy(CEA)，SO as to provide a theoretic evidence for the clinical observa— tion．Methods Hemodynamics examinations were performed in patients in the morning 7 days，3 days and 1 day before CEA and 1 week after CEA．The statistical analysis of hemodynamics data was performed before and after CEA．Results There was no significant difference in SBP，DBP and MBP before and after CEA(PO．O5)；there was significant difference in SBp，DBp and MBp before and after CEA(PO．01)．SBp，DBp and MBps had a rising tendency in 71．0％ of the pa— tients．There were 38．7％ of patients had disturbed heart rate，which gradually recovered to the normal leve1．Conclusion There are obvious changes in hemodynamics in carotid stenosis patients before and after CEA．The inspection of hemodynamics should be carried out to prevent the com— plications． Key words：carotid stenosis；endarterectomy；hemodynamics；monitoring
