第 3l卷第 2期 同济 大 学 学 报 (医 学 版 ) Vo1．3lNO．2 2010年 4月 JOURNALOFTONGJIUNIVERSITY(MEDICALSCIENCE) Apr．，2010 doi：10．3969~．issnlO08—0392．2010O．1．02．028 ·临床 研 究 · 鼻内镜手术治疗霉菌性鼻一鼻窦炎分析 郑 炯 ．马兆鑫 (同济大学 附属东方 医院耳鼻咽喉科 ，上海 200120) 摘【要】目的 探讨霉菌性鼻一鼻窦炎患者的临床表现及其诊断、治疗方法。方法 总结同济大学附属东方医院耳 鼻咽喉科 2000年 1月至 2007年 6月收治的3l例霉菌性鼻一鼻窦炎患者的临床资料 ，分析其临床表现、CT资料及 手术方法与疗效。结果 CT对霉菌性鼻一鼻窦炎的诊断和治疗有很高的价值 ，其特征性表现为病变窦腔密度不均匀 增高，内有局灶性点状、絮状、砂粒状、小斑片状高密度影 ，可有窦壁骨质破坏。所有患者均行鼻 内镜手术治疗 ，术后 定期冲洗 、清理术腔 ，随访 12～18个月，均获治愈。结论 霉菌性鼻一鼻窦炎以上颌窦多发，筛窦、蝶窦次之 ，其 CT表 现有特异性，鼻 内镜手术可达到根治和微创 的临床效果，关键是彻底清除病灶 ，建立足够的鼻腔鼻窦通气引流通道 ， 从而避免或减少复发 。 【关键词】鼻一鼻窦炎；霉菌性；鼻内镜术 【中图分类号】R765．4 文【献标志码】A 【文章编号2010)02～0103—04 Clinicalanalysisof31caseswithfungalrhino—sinusitis treatedbyendoscopicoperation ZHENG Jiong，MA Zhao—xin (Dept．ofOtorhinolaryngology，EastHospital，Ton~iUniversitySchoolofMedicine，Shanghai200120，China) A【bstract】Objective Tostudytheclinicalmanifestationofthepatientswithfungalrhino— sinusitis，and itsdiagnosisand treatment．M ethods Theclinicaldata and information of31cases with fungalrhino—sinusitisfrom January 2000 to June 2007were summarized，and their clinical manifestation，CT features，operationmethodsandcurativeeffectswereanalyzed．Results CT hada valuablemethodfordiagnosisandrteatmentoffungalrhino—sinusitis．ThemajorCTfindingswere heterogeneousdensityofsinusalcavitywith smallcalcification appeared asspots，floccose，sand—like orpatchy shadows，and localbone destruction．Allpatientswere treatedwith endoscopic technique andcuredafter12～18monthsfollowed—up．Conclusion M axillarysinusesaredamagedmoreoften than ehtmoid sinusesand sphenoid sinuses．Ith
