精品论文 参考文献 多层螺旋CT增强扫描在主动脉夹层诊断中的价值分析 淮安市淮安区淮安医院CT室 江苏淮安 223200 【摘 要】目的：探讨多层螺旋CT增强扫描在主动脉夹层诊断中的价值，为该疾病后续诊断、鉴别诊断及治疗提供较好的信息资料。方法：对我科2010年1月-2015年6月间35例主动脉夹层患者的多层影像CT平扫及增强扫描资料进行回顾性分析。结果：35例主动脉夹层按照DeBeKey分型，Ⅰ型：夹层源于升主动脉近端，延伸至降主动脉13例；Ⅱ型：夹层局限于升主动脉10例；Ⅲ型：夹层起自主动脉弓降部以远，局限于胸主动脉（Ⅲ型甲）7例，或累及腹主动脉（Ⅲ型乙）5例。CT表现有如下特征：①病变主动脉增宽；②主动脉腔内有稍弯曲的低密度线状内膜片影；③撕裂的内膜片将主动脉分为真/假两腔，其内CT值相同或不相同；④多数真腔小而假腔大。结论：多层螺旋CT增强扫描可快速的明显主动脉夹层的诊断，CT血管成像可更加清晰、直观地显示主动脉夹层的形态、范围，CT检查对本病的诊断、治疗方案的选择、疗效观察均起到重要指导意义。 【关键词】主动脉夹层；多层螺旋CT扫描；诊断 [Abstract] Objective：To investigate the value of multislice spiral CT scan in the diagnosis of aortic dissection，provides better information for subsequent disease diagnosis，differential diagnosis and treatment. Methods：The subjects in January 2010 - June 2015 between the multilayer image 35 cases of patients with aortic dissection plain and enhanced CT scans were retrospectively analyzed. Results：35 patients with aortic dissection according DeBeKey type，Ⅰ type：ascending aortic dissection from the proximal descending aorta extending to 13 cases；Ⅱ type：ascending aortic dissection limited to 10 cases；Ⅲ type：from the sandwich from Beyond descending aortic arch，thoracic aorta limited（Ⅲ Type A）7 cases，or involving the abdominal aorta（Ⅲ type B）5 cases. CT manifestations of the following characteristics：① diseased aortic widened；② aortic cavity slightly curved linear low density shadow intimal flap；③ aortic intimal flap tearing into true / false two chambers，which CT values within the same or different；④ most true lumen of small and large false lumen. Conclusion：Spiral CT scanning can be quickly enhanced diagnosis of aortic dissection obvious，CT angiography can be more clear，intuitive display form of aortic dissection，scope，CT examination of choice for the diagnosis，treatment regimen，efficacy Observation has played an important guiding significance.
