精品论文 参考文献 腮腺淋巴上皮病变影像学表现（病例报道及文献回顾） 贾传海 蒯新平 丁庆国 陆永明 张芬芬（通讯作者） 　　（江苏省常熟市第二人民医院影像中心 江苏 苏州 215500） 　　【摘要】 目的：探讨腮腺淋巴上皮源性病变影像学表现，以提高对本病的认识。方法：结合文献回顾性分析经临床或病理证实的9例腮腺淋巴上皮病变的影像表现。7例行MR平扫及增强，2例患者行CT平扫。结果：9例腮腺淋巴上皮源性病变均发生于腮腺，其中淋巴上皮癌3例，淋巴上皮瘤、鼻咽癌腮腺转移、原发恶性淋巴瘤各1例，Sjogren综合征3例。病理学及影像表现相似，但Sjogren综合征、恶性淋巴瘤具有一定特征。结论：影像学对腮腺淋巴上皮源性病变具有重要诊断及鉴别诊断价值，并可反映腮腺淋巴上皮源性病变之间的转归关系。 　　【关键词】 腮腺；淋巴上皮病变；X 线计算机体层摄影术；磁共振成像 　　【中图分类号】R730.4 【文献标识码】A 【文章编号】2095-1752（2015）22-0018-02 　　Radiologic findings of the lymphoepithelial lesions in parotid(cases report and literature review) 　　Jia Chuanhai，Kuai Xinping, Ding Qingguo，Lu Yongming，Zhang Fenfen. 　　Department of Radiology, Changshu No.2 Peoples Hospital, Changshu 215500, China 　　【Abstract】 Objective To explore the radiographic features of lymphoepithelial lesions in parotid gland and increase radiologists awareness of these lesions，facilitating prospective diagnosis. Methods 9 patients with lymphoepithelial lesions were reviewed retrospectively to determine whether any characteristic radiographic findings, and the relevant literatures were reviewed．Routine CT(2 cases)and MR(7 cases)，contrast-enhanced MR(7 cases)were performed. Results The lesions were all located in the s the parotid gland, including lymphoepithelial carcinoma(n=3), lymphepithelioma(n=1), malignant lymphoma(n=1), metastatic tumor from the nasopharyngeal carcinoma(n=1),and Sjogren syndrome(n=3).These lesions pathological findings were homoioplastic. The radiographic findings were complic,but the Sjogren syndrome and malignant lymphoma have some features. Conclusions Radiologic findings of the lymphoepithelial lesions in parotid gland have some significant value in diagnosis and differential diagnosis. 　　【Key words】 Parotid gland；lymphoepithelial lesion；Computerized tomography；Magnetic resonance imaging 　　腮腺淋巴上皮源性病变在组织学上起自于淋巴上皮组织，具有一定共同特征，相互间有一定转归或共同病因，包括淋巴上皮癌、淋巴上皮瘤、原发恶性淋巴瘤、Sjogren综合征及鼻咽癌转移，搜集本院9例经临床或病理证实的淋巴上皮源性病变并结合文献回顾探讨其影像特点及其诊断策略。 　　1.资料和方法 　　1.1
