妊娠期糖代谢异常对妊娠结局影响 　　[摘要] 目的 探讨妊娠期糖代谢异常对妊娠结局的影响。方法 对70例妊娠期糖代谢异常孕妇进行回顾性分析,其中妊娠期糖尿病(GDM)31例,妊娠期糖耐量受损(GIGT)39例,比较GDM、GIGT与血糖正常孕妇的高危因素和母婴结局。结果 GDM组、GIGT组与对照组年龄30岁、体重指数≥24、不良孕产史、糖尿病家族史等高危因素比较,差异有统计学意义(P0.05)。GDM组和GIGT组的新生儿窒息、低血糖、高胆红素血症的发生率均高于对照组,GIGT组新生儿低血糖发生率与对照组比较差异有统计学意义。GDM有3例胎死宫内,而GIGT组与对照组无胎死宫内病例。结论 对GDM和GIGT应同样重视,做到早筛查、早诊断、早治疗,减少母儿并发症的发生。 　　[关键词] 妊娠期糖代谢异常;妊娠期糖尿病;妊娠期糖耐量受损;妊娠结局 　　[中图分类号] R714.256[文献标识码] A [文章编号] 1673-9701(2010)12-26-03 　　 　　Influence of Abnormal Glucose Metabolism during Pregnancy on Pregnancy Outcome 　　LIN YuanZHONG HongzhuLI Zhuohua 　　Shantou Municipal No.2 People’s Hospital,Shantou 515000,China 　　 　　[Abstact] Objective To evaluate the influence of abnormal glucose metabolism during pregnancy on pregnancy outcome. Methods A retrospective study was conducted on 70 pregnant women with abnormal gestational glucose metabolism,which were classified into three groups: gestational diabetes mellitus group(31 cases of GDM),gestational impair glucose tolerance group(39 cases of GIGT)and control group. The high risk factors and pregnant outcomes of GDM,GIGT and normal glucose metabolism were compared. Results Statistic significance was found in the high risk factors such as age over 30,weight index≥24, abnormal child-bearing history and family history of diabetes between the GDM and GIGT groups and the control group(P0.05). The morbidity of new born asphyxia,hypoglycemia and hyperbilirubinemia in the DGM and GIGT groups was higher than that in the control group. Statistic significance was found when we compared the morbidity of new born hypoglycemia in GIGT group with that in the control group. Three cases of intrauterine fetal death were found in GDM group,while no cases of intrauterine fetal death were found in GIGT group and the control group. Conclusion We should deserve the same attention to GDM and GIGT and ensure early screening,early diagnosis and early treatment to reduce the incidence of complications of the mothers and fetus. 　　[Key words]
