复合高压水囊前列腺止血系列导管改良及应用 　　【中图分类号】R699【文献标识码】A【文章编号】1005～2720(2010)08 ～ 05 ～ 02 　　【摘要】目的 研究改良的复合高压水囊前列腺手术止血导管在耻骨上经膀胱前列腺切除术和前列腺电切术中的止血效果。方法 耻骨上经膀胱前列腺切除术中置复合高压水囊前列腺手术止血导管压迫腺窝同步行腺窝缘缩缝止血及前列腺电切术单用止血导管行止血治疗。结果 本组136 例,行耻骨上经膀胱前列腺切除术 46 例和前列腺电切术 90 例,止血效果可靠,均未经输血治疗,手术出血得到明显控制。结论 改良的二代前列腺手术系列止血导管在继承一代止血导管止血效果的同时,使用更方便。 　　【关键词】前列腺增生症;高压水囊;前列腺导管;止血 　　The improvement and clinical application of a prostatic hematostatic catheter with highpressurehyponome 　　(report of 136 cases) 　　Zhao HongboAi PeixingLi haifengJiang hanshengYang jieHu naigang 　　(DepartmentofUrology, PingduPeople′sHospital ShandongProvinca,Pingdushandong,266700) 　　【Abstract】ObjectiveTo study the stopping bleeding effect of the improved complex prostatichematostatic catheter with high pressure hyponome on the operation of suprapubic transvesical prostatectomy and transurethral prostatae electrotomy. Methods The complex prostatichematostaticcatheter was placed at the prostatecavity associated withcoarctation along the edge of the cavity to stoppingbleeding in suprapubic transvesical prostatectomy,and usingcomplexprostatic catheter only to stoppingbleeding in transurethral prostataeelectrotomy. Results 136cases, 46 casesweretreated through suprapubic transvesical prostatectomy,90 cases were treated through transurethral prostataeelectrotomy. The bleeding effect wasreliable. Therewasnoneed to transfusion in anycase. Bleeding during and after operation was obviously controlled.Conclusion The second generation improved hematostatic prostatic catheter withhigh pressurehyponome succeed the reliable bleeding effect of the first generation,and more convenient. 　　【Key words】Benignprostatichyperplasia;highpressurehyponome;prostatic catheter;Stoppingbleeding 　　 　　前列腺增生(BPH)是老年男性常见病和多发病,经尿道前列腺切除术(TURP)及耻骨上经膀胱前列腺摘除术(TVP)是目前治疗前列腺增生的主要方法。术中和术后前列腺窝出血一直是临床经常碰到的难题。我院应用自行设计的复合高压水囊前列腺手术止血导管,在经膀胱前列腺摘除术和TURP术后止血中取得了良好的效果,最近2年,我们对止血导管进行了改良设计,取得了满意的效果。现报告如下。 　　1 复合高压水囊前列腺手术止血导管 　　软导管,主体17F,长40cm。四腔:导尿腔、两高压止血水囊腔、冲洗腔。高压止血水囊:位于导尿腔开
