妊娠期糖尿病治疗对妊娠结局安全性影响 　　doi：10.3969/j.issn.1007-614x.2014.20.29 　　摘 要 目的：探讨妊娠期糖尿病规范治疗对妊娠结局安全性的影响。方法：2011年10月-2013年10月收治妊娠期糖尿病患者100例，将血糖控制达标者设为观察组，共70例，未达标者设为对照组，共30例，比较两组妊娠结局。结果：血糖有效控制70例，造成血糖控制无效的原因与医护依从差，运动、饮食、用药均未按医嘱严格执行相关。两组平均年龄、孕周、GDM血糖水平方面比较差异无统计学意义（P0.05）。对照组妊娠期高血压病、剖宫产、羊水过多、产后出血、胎膜早破、妊娠期并发症发生率均高于观察组，巨大儿、新生儿低血糖、胎儿窘迫、早产围生儿并发症率也高于观察组，差异均有统计学意义（P0.05）。结论：妊娠期糖尿病需早期明确诊治，制定有效方案及时治疗，以保障母婴健康，同时需针对危险因素加强防控，以全面提高产科质量。 　　关键词 妊娠期糖尿病 妊娠结局 安全性 影响 　　Effects of gestational diabetes mellitus treatment to the safety of pregnancy outcome 　　Min Fuchun 　　Department of Obstetrics and Gynecology，the Maternal and Child Health Hospital of Ezhou City，Hubei 436099 　　Abstract Objective：To investigate the effects of gestational diabetes mellitus standar treatment to the safety of pregnancy outcome.Methods：100 pregnancy in gestational diabetes were selected from October 2011 to October 2013.The blood glucose control standard of 70 cases were set to the observation group.30 cases were not to be standard set as the control group.We compared the pregnancy outcome of two groups.Results：The blood glucose of 70 cases were controled effectively.The cause of an invalid glycemic control were poor medical compliance，exercise，diet，medication not according to the prescribed strict implementation.The mean age，gestational weeks and blood glucose level of GDM of the two groups had no difference（P0.05）.The hypertensive disorder complicating pregnancy，cesarean delivery，polyhydramnios，postpartum hemorrhage，premature rupture of membranes and other complications during pregnancy of the control group were higher than those of the observation group.The macrosomia，neonatal hypoglycemia，fetal distress，perinatal complications of preterm birth rate were also higher than that of the observation group.All of the differences had statistical significance（P0.05）.Conclusion：The diagnosis and treatment of gestational diabetes mellitus should be made clear early，and take an effective programs for the treatment timely
