成都中医药大学硕士研究生毕业论文 成都中医药大学硕士研究生毕业论文 PAGE PAGE 1 目的： 中文摘要 运用中医基础理论阐述肾病综合征合并高凝状态的产生，根据辨证施治，制 定诊疗方案，并通过临床试验观察，客观评价其疗效，从而更好地指导临床。 方法： 总结古代医家对小儿肾病综合征合并高凝状态的认识，参考现代文献研究， 结合导师临床经验，提出小儿肾病综合征合并高凝状态的产生乃毒瘀胶滞所致。 依此病因病机创制银板逐瘀汤，它通过清热解毒，活血化瘀来治疗小儿肾病综 合征合并高凝状态,弥补过去认识中的不足（大多单以活血化瘀之法）。本研究采 用设计、实施及结果分析和解释等多方面与国际接轨的实效性对照试验，进一步 评价银板逐瘀汤治疗小儿肾病综合征合并高凝状态的临床疗效，为治疗小儿肾病 综合征合并高凝状态提供更科学的治疗方案。收集病例 60 例，用西医的治疗方 案作对照组，治疗组在对照组基础上加服银板逐瘀汤加减煎液，治疗 4 周后，进 行治疗组自身前后及治疗组与对照组治疗后在疾病疗效和症候疗效的比较。 结果： （1）在治疗蛋白尿、血尿、精神、面色黧黑或晦暗、咽喉肿痛、皮肤不泽 或肌肤甲错、肢体麻木等中医症候及舌苔、脉象方面，治疗组和对照组治疗后组 间及组内比较均有统计学意义（ p0.05）。（2）疾病疗效：治疗组总有效率 93.33 ％；阳性对照组总有效率 83.33％，两组比较有统计学意义（p0.05）。（3）症 候疗效统计：治疗组总有效率 96.67％；阳性对照组总有效率 80％，两组比较有 统计学意义（p0.05）。（4）高凝状态疗效：治疗后两组 Fbg，D-D 显著下降 （p0.05），Fbg，D-D 治疗组变化较对照组更显著（p0.05）。 结论： 小儿肾病综合征合并高凝状态的产生与毒瘀病理关系密切，从毒瘀胶滞辨治 小儿肾病综合征合并高凝状态，运用银板逐瘀汤加减煎液结合西医治疗本病疗效 确切，有其独特的优势，值得进一步研究并应用于临床。 关键词： 毒瘀胶滞 小儿肾病合并高凝状态 银板逐瘀汤 Objective: Abstract According to basic theory of TCM ,described the occurrence of nephrotic syndrome with hypercoagulable state,according to syndrome differentiation,development of treatment programes,and through observation of clinical trials,objective evaluation of its efficacy,so as to better guiding clinical.
