老年2型糖尿病合并慢性阻塞性肺疾病患者胰岛素抵抗及胰岛功能分析 　　doi：10.3969/j.issn.1007-614x.2014.11.16 　　摘 要 目的：讨论老年2型糖尿病合并慢性阻塞性肺疾病（COPD）患者胰岛素抵抗及胰岛功能情况。方法：收治老年2型糖尿病患者90例，随机分组，单纯性2型糖尿病的患者44例为对照组；COPD合并2型糖尿病患者46例为观察组，另外有25例健康志愿者为空白对照组。对三组研究对象的空腹及早餐后2小时血糖、糖化血红蛋白、空腹C肽、餐后2小时C肽、空腹胰岛素、餐后2小时胰岛素等指标进行观察与比较，并计算胰岛素抵抗指标（HOMA-IR）。结果：餐后2小时，观察组的平均C肽值1.36mmol/L，而对照组和空白对照组分制为4.01mmol/L和3.99mmol/L，观察组的数值明显比其他两组的数值偏低（P0.05）；观察组与对照组和空白对照组的HOMA-IR相比，观察组的HOMA-IR的平均值5.0，对照组和空白对照组的平均值2.9、2.8。因此，观察组的HOMA-IR值明显升高，胰岛细胞功能减退（P0.05）。结论：2型糖尿病合并COPD患者出现明显的胰岛素抵抗及胰岛功能明显降低，本研究对临床具有指导意义。 　　关键词 老年2型糖尿病 COPD 胰岛素抵抗 胰岛功能 　　Analysis of islet function and insulin resistance in elderly patients with type 2 diabetes mellitus complicated with COPD 　　Ping Jinxiu 　　Internal medicine，the Traditional Chinese Medicine Hospital of Jiyang County，Shandong 250000 　　Abstract Objective：To explore the islet function and insulin resistance in elderly patients with type 2 diabetes mellitus complicated with COPD.Methods：90 elderly patients with type 2 diabetes were selected.44 cases of patients with type 2 diabetes mellitus were the control group；46 cases of diabetic patients complicated with COPD were the observation group.25 healthy volunteers were the normal control group.Fasting and after breakfast 2 hours blood glucose，glycosylated hemoglobin，fasting C peptide，2 hours postprandial C peptide，fasting insulin，2 hours postprandial insulin of the three groups were compared，and insulin resistance index（HOMA-IR） was calculated.Results：At 2 hours after the meal，the average C peptide value of the observation group was 1.36mmol/L，control group and blank control group were 4.01mmol/L and 3.99mmol/L，the observation group was significantly lower than the other two groups（P0.05）.The HOMA-IR mean value of observation group was 5，the average value of control group and blank control group were 2.9 and 2.8，the HOMA-IR of observation group was increased obviously，the function of islet cells was decreased（P0.05）.Conclusion：Type 2 diabetic patients with COPD app
