早期血乳酸水平检测在重症肺炎患者中临床意义 　　[摘要] 目的 探讨早期血乳酸水平检测在重症肺炎患者中的临床意义。 方法 选取本院2011年5月～2014年5月收治的100例重症肺炎患者作为研究对象，根据存活或死亡分为观察组（生存患者）和对照组（死亡患者）。比较两组入院时、入院后6 h、入院后24 h的血乳酸水平及入院6 h的乳酸清除率，记录不同乳酸清除率水平患者的死亡率。 结果 观察组入院时、入院6 h和入院24 h的血乳酸水平显著低于同期对照组，差异有统计学意义（P0.05）。观察组入院6 h的乳酸清除率显著高于对照组，差异有统计学意义（P0.05）。高乳酸清除率患者的死亡率显著低于低乳酸清除率患者，差异有统计学意义（P0.05）。 结论 检测重症肺炎患者的血乳酸水平有助于了解患者的预后，血乳酸清除率低的患者死亡率高。 　　[关键词] 乳酸清除率；重症肺炎；急诊 　　[中图分类号] R563.1+9 [文献标识码] A [文章编号] 1674-4721（2015）06（c）-0127-03 　　[Abstract] Objective To explore the clinical significance of early blood lactic acid level test in patients with severe pneumonia. Methods 100 patients with severe pneumonia from May 2011 to May 2014 in our hospital were selected and divided into the observation group （surviving patients） and the control group （deceased patients） according to live or die.The levels of lactic acid in arterial blood upon admission to the hospital，6 hours after admission and 24 hours after admission，the lactic acid clearance rate at 6 hours after admission in two groups was compared.Mortality of patients with different levels of lactic acid clearance rate was recorded. Results The levels of blood lactic acid upon admission to the hospital，6 hours after admission and 24 hours after admission in the observation group was lower than that in the control group during the corresponding period，with significant difference（P0.05）.The lactic acid clearance rate at 6 hours after admission in the observation group was higher than that in the control group，with significant difference（P0.05）.The mortality rate of patients with high lactate clearance rate was lower than that of patients with low lactate clearance rate，with significant difference（P0.05）. Conclusion The blood lactic acid level test in patients with severe pneumonia helps understand patients prognosis，and the patients with lower clearance rate of blood lactic acid have higher mortality rate. 　　[Key words] Lactate clearance rate；Severe pneumonia；Emergency treatment 　　重症肺炎患者病情较重，可合并多器
