瑞芬太尼复合丙泊酚在老年无痛胃镜诊疗中安全性研究 　　【摘 要】目的 探讨瑞芬太尼复合丙泊酚在老年无痛胃镜诊疗中的安全性。方法 选取我院消化内镜室2013年10月至2015年3月应用瑞芬太尼复合丙泊酚行无痛胃镜诊疗的242例老年患者作为研究对象，按照随机原则将其分为A组（80例）、B组（80例）、C组（82例），三组患者静脉注射瑞芬太尼的剂量为0.25μg/kg、0.5μg/kg、0.75μg/kg，随后给予三组患者丙泊酚的剂量为0.5～1.5mg/kg，麻醉起效后，对三组患者进行胃镜诊疗，观察患者的各项生命体征和不良反应情况。结果 三组患者均顺利完成无痛胃镜诊疗。三组患者对麻醉效果都比较满意。相较于诊疗前，三组患者诊疗中的平均动脉压（MAP）、心率（HR）均有所下降，具有统计学差异（Plt;0.05）。在SpO2变化、不良反应方面，A组、B组优于C组，具有统计学差异（Plt;0.05）。结论 在老年无痛胃镜诊疗中，应用合适剂量的瑞芬太尼，同时联用丙泊酚，不仅安全性高，而且患者苏醒速度快，麻醉镇痛效果理想，值得广泛应用和推广。 　　【关键词】瑞芬太尼;丙泊酚;老年;胃镜诊疗;安全性 　　Abstract：Objective To investigate the safety of remifentanil combined with propofol in painless gastroscopy in elderly patients.Methods 242 cases of elderly patients of digestive endoscopy room in our hospital from 2013 October to 2015 March application of remifentanil combined with propofol for painless gastroscopy as the research object，according to randomly divided into group A（80 cases），B group（80 cases），C group（82 cases），three groups of patients with intravenous remifentanil the dose of 0.25，0.5，0.75 g/kg，then the three groups of patients were given propofol at a dose of 0.5 ～ 1.5mg/kg，after the onset of anesthesia，endoscopic diagnosis and treatment of three groups of patients，observe the vital signs of patients and adverse reactions.Results three patients were successfully completed the painless gastroscopy.Three groups of patients for anesthesia are more satisfied with the results.Compared with before treatment，three groups of patients with mean arterial pressure（MAP）in the diagnosis and treatment，heart rate（HR）decreased，with statistical difference（Plt;0.05）.Changes in SpO2，adverse reactions，A group，B group than in C group，with statistical difference（Plt;0.05）.Conclusions in elderly patients with painless gastroscopy，application of remifentanil，combined with propofol，not only has high safety，and thepatients recovery quickly，analgesic effect is ideal，it is worthy of wide application and promotion. 　　Keywords：Remifentanil Propofol The elderly Gastroscopy S
