肺结核患者细胞免疫水平与肺功能相关性研究 　　【摘要】目的 通过对肺结核患者及健康对照者的细胞免疫水平和肺功能进行检测,观察肺结核患者不同程度的细胞免疫水平及肺功能状态,并将他们的细胞免疫水平及肺功能进行相关性分析。方法所有肺结核患者按病变程度分为重度、中度和轻度各25例。应用流式细胞仪检测75例各组肺结核患者和30例健康人群的外周血中CD4 、CD8及CD4 /CD8 值并进行比较;同时检测75例肺结核患者肺功能,进行各组别间比较，并与外周血中CD4 、CD8及CD4 /CD8 值进行相关性分析。结果 ①肺结核组与健康组比较,外周血中CD4 、CD8及CD4 /CD8 值3组数据均差异有统计学意义( P 0.05) 。③FEV1 %、MMEF%与CD8 细胞百分比呈显著负相关( P 0.05) ,与CD4 细胞百分比和CD4 /CD8 比值呈显著正相关( P 0.05) 。结论 肺结核发病与肺结核患者的细胞免疫水平下降有关。肺结核患者外周血T淋巴细胞亚群可能对其肺功能具有一定影响。 　　【关键词】肺结核 T淋巴细胞亚群 CD8 T淋巴细胞 CD4 T淋巴细胞 肺功能 　　中图分类号：R521 文献标识码：B 文章编号：1005-0515（2011）5-060-03 　　The correlation study of lung function and cellular immunity in pulmonary tuberculosis patients 　　Li Meiqing Li DanWang Yu 　　The Third People’s Hospital of Yichang, Yichang,Hubei 443003, China 　　【Abstract】Objective To explore the correlation of lung and cellular immunity in different pulmonary tuberculosis patients. Methods The percentage of CD4, CD8 in peripheral blood of 75 pulmonary tuberculosis patients (severity 25 cases, moderate 25 cases, and mild 25 cases) and 30 health volunteer were analyzed by cytometry, meanwhile lung function of 75 pulmonary tuberculosis patients were measured. Results ①The percentage of CD4, CD8 and ratio of CD4/CD8 were significant different both between tuberculosis group and control group and among tuberculosis patients (severity, moderate, and mild) (P ?0.05) ; ②FEV1, FEV1%, and MMEF% of severity and moderate tuberculosis patients were significant lower than those of mild tuberculosis patients and normal reference value; ③ In tuberculosis patients, FEV1%and MMEF% were significantly negative correlation of CD8 percentage (P 0.05), also significantly positive correlation of CD4 percentage and ratio of CD4/CD8 (P 0.05). Conclusions Pulmonary tuberculosis morbility maybe due to decreasing of cellular immunity of patients; T lymphocyte subgroup in peripheral blood could affect the lung function of pulmonary tuberculosis patients. 　　【Key words】pulmonary tuberculosis T lymphocyte subgroup CD8
