重症脑卒中患者早期肠内肠外和个体化营养治疗对比分析
[摘要] 目的 比较分析重症脑卒中患者早期肠内、肠外与个体化营养支持的临床价值。 方法 收集自2009 年1月~2014年1月收治的60例我院ICU的重症脑卒中患者作为研究对象, 根据其营养支持方法的不同分为肠内营养组、肠外营养组与个体化营养组,比较三组患者治疗前后营养指标变化、并发症发生率等。 结果 三组患者治疗后血清总蛋白(TB)、血清白蛋白(ALB)、血清前白蛋白(PA)均明显改善(P0.05);个体化营养治疗组营养改善明显优于肠外营养组和肠内营养组,且并发症少于肠内营养组(P0.05);三组患者并发症发生率分别为70.0%、45.0%、10.0%,差异有统计学意义(P0.05)。 结论 个体营养治疗支持能够有效改善重症脑卒中患者的营养状况,减少并发症发生,减轻患者家庭经济负担,同时可促进患者尽早康复。

[Abstract] Objective To assess the effect of early enteral, parenteral nutrition and individualized support for severe stroke patients. Methods A total of 60 severe stroke patients were investigated from January 2009 to January 2014 in our Hospital ICU department. They were divided into enteral nutrition group, parenteral nutrition support group and individualized nutrition therapy group according to different nutritional support methods. Serum nutritional indexes(TB, ALB and PA) were measured before and after treatment. Results The levels of TB, ALB and PA were higher in all groups of patients after treatment(P0.05); they were improved significantly in individualized nutrition therapy group compared to parenteral nutrition and enteral nutrition group, while the complications rates of individualized nutrition therapy group was significantly lower (P0.05); The complication rates of three groups(parenteral nutrition group, enteral nutrition group and individualized nutrition therapy group) were 70.0%, 45.0%, 10.0% respectively. There was significant difference (P0.05). Conclusion Individual nutritional therapy support can promote early recovery nutritional status of patients with severe stroke, reduce related complications and the financial burden on the patients family, and also can promote early rehabilitation of patients.
