58例肛周脓肿细菌培养和药敏试验分析 　　【中图分类号】R657.15【文献标识码】A【文章编号】1005-2720(2010)05 - 05 - 03 　　【摘要】目的 探讨肛周脓肿病原菌及药敏试验,指导临床合理使用抗生素。方法 对近5年在遂宁市中心确诊为肛周脓肿患者58例的脓液进行细菌培养,选取菌株较多的前四种细菌,对16种常用抗菌药物进行药敏试验。 结果 细菌检出率100%,多数为混合感染占67.2%,检出菌株最多的细菌依次是:大肠埃希氏菌27株,阴沟肠杆菌23株,金黄色葡萄球菌20株,铜绿假单胞杆菌16株,金黄色葡萄球菌对万古霉素敏感,对三代头孢有一定耐药性;大肠埃希氏菌、阴沟肠杆菌对亚胺培南敏感,对三代头孢菌素和丁胺霉素,喹诺酮类较为敏感,铜绿假单胞杆菌耐药品种较多。结论 肛周脓肿多为混合感染,治疗前应尽可能进行细菌培养和药敏试验,治疗时使用三代头孢等广谱抗生素,并联合使用抗厌氧菌药物。 　　【关键词】肛周脓肿;细菌培养;药敏试验 　　The analyses of 58 perianal abscess germiculture and susceptibility test 　　Liu Xi 　　(Department of hemorrhoids and fistula,the central hospital of Suining, Sichuan 629000) 　　【Abstract】Objective Destination Probe the perianal abscess’s pathogenic bacteria and susceptibility test, to guidance clinic use antibiotic rationally .Methods We make bacterial culture with liquor puris which get from 58 patients who have been final diagnosis to be perianal abscess by Central hospital of Suining, choose the first four kind major strain, and then use 16 kind of commonly used antibiotics to doing susceptibility test. Results The germ detection rate is 100%, mixed infection account 67.2%,, the detection strains sequentially list by quantity was: large intestine escherichia 27 strain,bacillus cloacae 27 strain, staphylococcus aureus 20 strain, pseudomonas aeruginosa 16 strain. staphylococcus aureus is sensitive to vancocin, and exist drug tolerance to the third generation cephalosporin to some degree;large intestine Escherichia、bacillus cloacae is sensitive to imipenem, and keep sensitive to the third generation cephalosporin、butylamine-mycin、quinolones; Pseudomonas aeruginosa is drugfast to a lot of species. Conclusion Most perianal abscess is mixed infection, We’d better try to make germiculture and susceptibility test before treating, use the broad-spectrum antibiotic as the third generation cephalosporin and so on, and association use the antianaerobic-microbacterial drug . 　　【Key words】Perianal abscess;Germiculture;Susceptibility test 　　 　　我院肛肠科
