竺!!翌查竺 竺!!翌查竺 一——————!主竺苎竺苎一 黄斑水肿的光学相干断层扫描分析 研究生 谢娟 导师 成霄黎 教授 摘 要 目的：观察黄斑水肿(macular edema)的光学相干断层扫描 (optical coherence tomography，OCT)图像特征，测量黄斑水肿患 眼的黄斑中心凹厚度，并探讨黄斑中心凹的厚度与最佳矫正视力 之间的关系。方法：对47例54只经直接、间接检眼镜、三面镜 或荧光素眼底血管造影(fundus fluorescein angiogrphy，FFA)检查 诊断或拟诊为黄斑水肿的患眼进行OCT检查，行通过黄斑中心凹 的水平或垂直方向线性扫描，测量黄斑中心凹的厚度，并与其最 ， 佳矫正视力进行相关分析。／结果：54只黄斑水肿的患眼中，20只 眼表现为黄斑区视网膜海绵样肿胀(sponge—like retinal swelling)， 占37．1％；26只眼表现为黄斑囊样水肿(cystoid macular edema)， 占48．1％；8只眼表现为浆液性视网膜神经上皮脱离(serous retinal detachment)，占14．8％。 黄斑水肿患眼的黄斑中心凹厚度较正常 眼显著增厚。黄斑水肿患眼的黄斑中心凹厚度与其最佳矫正视力 、／ 呈负相关(r一0．569，p=O．000)0结论：黄斑水肿的OCT图像主要 包括视网膜海绵样肿胀、黄斑囊样水肿及神经上皮浆液性脱离。 黄斑水肿患者的黄斑中心凹厚度明显增厚，黄斑中心凹厚度越厚， 视力越差。 关键词：黄斑水肿光学相干断层扫描 ±!曼竺查竺——————————竺兰!!!!三一Optical ±!曼竺查竺——————————竺兰!!!!三一 Optical Coherence Tomography of Macular Edema Abstract Objectire：To observe the characteristics of optical coberence tomog。8phY (OCT)in macular edema，measure the retinal thickness at the central fovea and study the relationship between the thickness and the visual acuity with best correction of affected eyes．Methods：Total of 47 cases(54 eyes)with ● clinical diagnosis of macular edema by direct，indirect ophthalmoscopy，three mirror contact lens，or fundus fluorescein angiography(FFA)were examined with OCT，the examination focused on the horizontal and vertical planes crossing the central fovea．we studied the correlation between retinal thickness at the central fovea and best．corrected visual acuity of affected eyes Result：In the 54 eyes，sponge-like retinal swelling was found in 20 eyes (37．10％)，macu Jar cystoid edema was found in 26 eyes(48．I％)，and serous retinal detachment was found in 8 eyes(1 4_8％)．The retinal thickness at the central fovea of the affected eyes was thicker than that of normal eyes．The statistical analysis showed there was a negative correlation between the thickness at the central fovea and best-corrected visual acuity of affected eyes．(r=一0．569，p=0．ooo)．Conclusion：There were three types of images of OCT in macular edema：spo
