血清胆碱酯酶水平对脓毒症患者病情判断的意义【摘要】目的探讨血清胆碱酯酶(CHE)水平对脓毒症患者病情严重程度评价的意义。方法观察2010年1月至2012年12月我院1CU收治的脓毒症患者40例，按 APACHEII分值分组，比较各组间血清CHE指标的差异，分析其与患者病情严重程度的相关性。结果血清CHE浓度在APACI1EII评分11-20分组(4318. 72±789. 18) 显著低于 W 10分组(9063. 80± 1365. 47, P0. 05), APACHEII评分21-29分组 (2074. 79 ± 588. 63 )显著低于 11-20分组(4318. 72 ± 789. 18, P0. 05),而 APACHE 11评分2 30分组(96 80 ± 172. 79 )明显低于21-29分组 (2074. 79±58 63, P0. 05),差异具有统计学意义。血清CHE水平与APACHEII 评分呈显著负相关(r=-0. 435, P0.01)o结论血清CI1E浓度与APACHEII评分具有较高的相关性，血清CHE浓度随APACHEII评分升高而降低，血清CHE可能是判断脓毒症患者病情严重程度较为简便，敏感的指标。【关键词】血清胆碱酯酶APACHEII评分脓毒症 Significance of serum cholinesterase level to the judgment of the condition of patients with sepsis 【Abstract 1 Objective To investigate the significance of serum cholinesterase level to the evaluation of the condition of patients with sepsis. Methods To observe 40 patiens with sepsis who were admitted in our hospital ICU from January 2010 to December 2012. Grouped according to APACHEII score.Compare the difference of semm cholinesterase among all groups.Analyze its relationship with the severity of patients9 condition.Results serum concentration of cholinesterase in APACHEII score of 11 to 20 group (4318.72 ± 789.18 ) were significantly lower than 10 group (9063.80± 1365.47, P0.05) ;21 to 29 group (2074.79±588.63) were significantly lower than 11 to 20 group (4318.72 ±789.18, P0.05) ;while the APACHEII score 30 group (968.80± 172.79) were significantly lower than 21 to 29 group (2074.79±588.63, P0.05 ) .difference was statistically significant. APACHEII score and serum cholinesterase level were significantly related to ratio ( r二一0? 435, PvO. 01). Conclusion serum concentration of cholinesterase and APACHEII score has obviously relationship, serum concentration of cholinesterase increased with APACHEII score at the same time, serum cholinesterase may be a simple and sensitive indicator of judging the severity of patients condition.
