提要 提要 少46,1994 目的：探讨凉血散瘀法外用治疗静脉性溃疡的临床疗效及其作用机 理，为静脉性溃疡的中医外治疗法奠定理论基础。 份法：临床观察，将60例静脉性溃疡患者随机分为两组，治疗组 40例、对照组20例，治疗组外用愈疡灵软膏创面换药，对照组外用大 黄油纱换药，两组均为每日换药1次，治疗4周。动物实验将60只大鼠 分两批进行，第一批采用肉芽肿模型，用于不同组别的肉芽组织生长状 况及形态组织学观察、测定肉芽组织中的碱性成纤维细胞生长因子 (bFGF)的水平、胶原纤维的密度值及DNA含量，第二批采用创伤模 型，用于观察不同组别的上皮组织生长状况。 结果：临床研究显示，愈疡灵软膏治疗静脉性溃疡，总有效率为 97．5％，对照组总有效率为75％，经统计学处理，有显著意义(P0．05)。 动物实验研究显示，愈疡灵软膏能促进成纤维细胞的增殖、DNA的复制 和新生毛细血管的形成及胶原合成，提高局部bFGF的水平，加速创面 肉芽组织形成及再上皮化速率，从而起到促修复作用。斗 结论：愈疡灵软膏是促进静脉性溃疡愈合的有效制剂。本研究为中 医外治疗法治疗静脉性溃疡提供了理论依据，为临床提供了一种新的有 效的治疗方法。 关键词：静脉性溃疡凉血散瘀愈疡灵软膏?临床研究)r实验研究 Research Research of Therapy“cool the blood and dissipate the blood stasis”in 4 Treatment如r Venous Ulcer Specialty：Peripheral Vascular Disease，Surgery Tutor：Professor HOH Yu fen Graduate：Liu Chun mei Abstract objective：The author explores clinical curative effect and the function mechanism of“cool the blood and dissipate the blood stasis’’on extemal treating venous ulcer，which establishes the theoretical basis of TCM external therapy on treating venous ulcer． Method：Clinical observation 60 patients with venous ulcer are divided into two groups at random．The treatment group f40 patientsl changes the dressing of wounded area with“yu yang ling ointment”，but the control group(20 patients) changes the dressing with rhubarb oily gauze Two groups change the dressing once a day and this therapy last four weeks．Animal experimental 60 mice are divided into twO batches．nle first batch of mice are adopted granuloma model．which are used to observed different groups granuloma tissue growth state and morphological histology，measure the level of basiciry fibroblast growth factor(bFGF)among the granuloma tissue and density of the collagenous fbre and content of DNA．The second batch ofice are adopted trauma model，which are used to observed the growth slate ofdifferent groups epithelical tissue． Result：The clinical result shows：the total effective rate oftreatment group and control group is 97 5 per and 75 per respectively，which are remarkably differ
