???? 三峡库区钩端螺旋体病血清流行病学调查 ???? ????摘　要　目的：摸清库区钩体疫源地的分布、人群钩体抗体水平及菌群分布。方法：显微凝集试验。结果：1998年从三峡库区7个疾病监测点采集健康人血清513份，钩端螺旋体(钩体)抗体阳性165人(阳性率32.16%)，各监测点阳性率从11.11%～50.00%不等。抗体阳性率与性别无关，而与年龄密切相关。库区人群含9群钩体血清抗体，以黄疸出血群为主。钩体血清菌群GMT值均不高。结论：库区人群钩体免疫水平低下，应加强钩体病的监测和防治工作。　　关键词　钩端螺旋体病； 血清流行病学； 抗体 Sero-Epidemiological Investigation of Leptospirosis in the ThreeGorge Reservoir Area Wu Guohui　Wang Weijun　Chen Daihong(Health and Antiepidemic Station of Chongqing,Chongqing 400042) 　　Abstract　In order to find out the main area and crowd infected by leptospirosis and the main epidemic leptospir serogroups in the three gorge reservoir area， 513 serum samples were collected from healthy people in 7 monitoring places in 1998 and leptospira antibody was examined. Results showed that 165 cases were positive (32.16%), from 11.11% to 50.00% in different areas. The infection was interrelated to age but not to sex. There were 9 serogroups of leptospira antibody identified in this study. Icterohaemorrhagiae serogroups were the dominant serogroup. All of the serogroups antibody in the throng of people in lower level. It suggests that the surveillance of leptospirosis need to be strengthened and a comprehensive strategy including effective management and control leptospirosis need to be developed in the three gorge reservoir area.　　Key words　Leptospirosis; Sero-epidemiology; Antibody　　为了解和掌握三峡库区钩体病的流行特征和规律，提高钩体病防治水平，控制本病流行。根据卫生部疾控发(1996)第10号文的附件《三峡坝区及库区疾病监测方案》中有关要求，我们于1998年6～9月在三峡库区7个疾病监测点进行了人群血清学监测。 1　材料与方法 1.1　调查地点　按“监测方案”选定7个疾病监测点(城市点：九龙坡、江北、万州；城镇点：巴南；农村点：丰都、长寿、渝北)，分别代表三峡库区不同地域情况。1.2　标本采集　1998年6～7月，各个监测点对监测人群按不同年龄、性别比例，在健康人群中随机抽样，静脉抽血2～3ml，分离血清，56℃水浴灭活30min，胶塞封口冻存，统一时间由专人进行血清学试验。 1.3　钩体菌株　将中国预防医学科学院流行病学微生物学研究所钩体室提供的我国13群15种型标准钩体菌株，接种于Korthof培养基中，28℃培养5～7d，取样作暗视野显微镜检查，每400倍视野不少于50条、运动活泼并无自凝现象者，作为抗原。1.4　试验方法　用显微镜凝集试验检测血清抗体，稀释度为1∶20、1∶40、1∶80、1∶160 4个稀释度，用生理盐水作对照。凡凝集滴度达1∶20?以上者为阳性。 2　结果 2.1　地区分布　7个监测点的513份健康人血清标本经显凝试验，阳性165份，阳性率32.16%，各地阳性率由10.81%～50.00%不等(表1)。 表1　7个监测点人群血清检测结果 监　测　点 检测 人数 阳性数 阳
