1.标准StandardAOP1

医院依据已建立的评估程序，进行院内所有照护病人的健康照护需求确认。Allpatientscaredforbythehospitalhavetheirhealthcareneedsidentifiedthroughanassessmentprocessthathasbeendefinedbythehospital.

2.目的Goal

对患者的评估是通过询问病史、体格检查、临床实验室检查、医技部门辅助检查等途径，对患者的心理、生理、社会、经济状况、病情严重程度、全身状况支持能力等的评估。医生对评估数据、信息进行分析和整合，全面把握患者基本现状和诊疗服务的需求，确保提供给每一位患者的医疗护理措施均基于患者相关心理、生理及社会需求、经济因素，以及物理、化验室检查和病史的初步评估。Patientassessmentsareconductedbyaskingpatient’smedicalhistory,performingphysicalexamination,clinicallaboratorytest,andsupportbyparamedicaldepartmentauxiliaryexaminationstoevaluatethepatient’spsychological,physical,social,economicstatus,severityofmedicalcondition,overallstatussupportingabilityetc.Physicianintegratesandanalysisassessmentdataandinformationtofullyunderstandthepatient’scurrentconditionandmedicalneeds.Thisistoassurethatthemedicalandnursingcareprovidedtothepatientisbasedonthepatient’sinitialassessmentresultsrelatedtotheirpsychological,physical,andsocialneeds,economicfactor,physiologicalandlaboratoryexam,andmedicalhistory.

3.范围Scope

所有患者均应经医院所建立的评估程序，确认其健康医疗护理的需求，包含所有到院的病人，包含住院、门诊病人。Allpatientsshouldbeevaluatedbythehospital’sassessmentprocesstoidentifytheirmedicalandnursingneeds.Allinpatientsandoutpatientsareincluded.
