术前输注支链氨基酸对老年人开腹手术围术期体温的影响

[摘要]:目的:观察老年人术前给予支链氨基酸对围术期体温的影响。方法:选择60例ASAⅠ或Ⅱ级,年龄60~84岁行择期腹部手术的患者,随机均分为2组,观察组(氨基酸组)术前1h给予支链氨基酸3.5ml/kg,对照组给予羟乙基淀粉溶液3.5ml/kg。分别记录两组入室时(T1)、诱导时(T2)、切皮时(T3)、诱导后1h(T4)、诱导后2h(T5)、术后2h(T6)的中心温度(食道温度),同时记录围术期寒战的发生率。结果:观察组术中、术后体温无明显变化,无术后低体温发生;对照组术中、术后体温较术前明显下降(P<0.05),与观察组相比有明显降低(P<0.05),对照组术后寒战的发生率显著高于观察组(P<0.05)。结论: 术前输注支链氨基酸有助于维护维护老年腹部外科老年患者的体温,并降低术后寒战的发生率。

[Abstract] Objective To observe the effect of amino acid solution on perioperation hypothermia in geriatric patients with abdominal surgery. Methods Sixty geriatric patients with ASAⅠ~Ⅱ were randomized into two groups: Group Amino Acid solution(AA)and Group Hydroxyethyl Starch(HES).Esophagus core temperature were periopaeratively monitored at OR admission, anesthesia induction, skin incision,1 hr and 2 hr after anesthesia induction. 2 hr after the end of operation. The incidence of perioperative shivering and sweating was recorded. Results Perioperative core temperature of patients in Group AA was maintained compared with preoperative time point,and postoperative hypothermia was not founded. The patients’ body temperature in Group HES was obviously decreased intra- and post- operatively compared with preoperative period (P0.05), and significantly lower than patients’ temperature in Group AA (P0.05). The incidence of postoperative shivering in Group HES was increased statistically compared with Group AA (P0.05). Conclusion Preoperative amino acid infusion is beneficial for maintaining the core body temperature in geriatric patients with abdominal surgery, and decreasing the incidence of postoperative shivering.
